中性粒细胞与淋巴细胞比值预测脓毒症患者预后的价值

摘要目的研究中性粒细胞与淋巴细胞比值在脓毒症患者预后状况预测中的价值。结果回顾性分析2017年7月～2019年3月东莞市茶山医院治疗的50 例脓毒症患者的临床资料，将发病后28 d 救治无效死亡的12 例患者作为病死组，将发病后28 d 救治成功生存的38 例患者作为生存组，比较两组的脓毒症严重状况、临床指标（包含血乳酸检测值、急性生理学和慢性健康状况评分系统评分值、中性粒细胞与淋巴细胞比值、降钙素原检测值、血小板数目检测值、白细胞数目检测值）。结果病死组的脓毒症休克占比高于生存组，脓毒症占比低于生存组，差异有统计学意义（P＜0.05）；病死组的血乳酸检测值、急性生理学和慢性健康状况评分系统评分值、中性粒细胞与淋巴细胞比值高于生存组，差异有统计学意义（P＜0.05）。两组患者的降钙素原检测值、血小板数目检测值、白细胞数目检测值比较，差异无统计学意义（P＞0.05）。结果采用中性粒细胞与淋巴细胞比值预测脓毒症患者预后状况具有重要价值。

Abstract：Objective To study the value of neutrophil to lymphocyte ratio in predicting the prognosis of sepsis patients.Methods The clinical data of 50 patients with sepsis treated in Chashan Hospital of Dongguan City from July 2017 to March 2019 were retrospectively analyzed.A total of 12 patients died of ineffective treatment at 28 days after the onset of sepsis were taken as the death group,and 38 patients who survived at 28 days after the onset of sepsis were taken as the survival group.The severity of sepsis and clinical indicators (including blood lactate test value,acute physiology and chronic health score system score value,neutrophil to lymphocyte ratio,procalcitonin test value,platelet count test value,white blood cell number test value) of the two groups were compared.Results The proportion of septic shock in the death group was higher than that in the survival group,and the proportion of sepsis in the death group was lower than that in the survival group,the difference was statistically significant (P＜0.05).The values of blood lactate,acute physiology and chronic health score system,neutrophil to lymphocyte ratio in the death group were higher than those in the survival group,and the differences were statistically significant (P＜0.05).There was no significant difference in procalcitonin,platelet count and white blood cell count between the two groups (P＞0.05).Conclusion The ratio of neutrophil to lymphocyte is of important value in predicting the prognosis of sepsis patients.
