Value of pulmonary alveolar lavage fluid Mycobacterium tuberculosis RNA combined with Xpert detection in the diagnosis of smear-negative pulmonary tuberculosis

Abstract:Objective To investigate the clinical value of pulmonary alveolar lavage fluid Mycobacterium tuberculosis RNA combined with Xpert detection in the diagnosis of smear-negative pulmonary tuberculosis.Methods A total of 348 patients with smear-negative pulmonary tuberculosis treated in the Department of Respiratory Medicine of Chronic Disease Prevention Hospital of Qingyuan City from February 2017 to October 2018 were selected as research subjects.All patients were detected by pulmonary alveolar lavage fluid Mycobacterium tuberculosis RNA combined with Xpert.Taken the culture of pulmonary alveolar lavage fluid Mycobacterium tuberculosis as the gold standard for diagnosis,the sensitivity,specificity,accuracy,Youden index,positive likelihood ratio and negative likelihood ratio of the combined detection and single detection of smear-negative pulmonary tuberculosis were compared.Results The sensitivity,specificity,accuracy,Youden index,and positive likelihood ratio of the combined detection of pulmonary alveolar lavage fluid Mycobacterium tuberculosis RNA and Xpert were significantly higher than those of single detection,the negative likelihood ratio was lower than that of single detection,and the differences were statistically significant (P<0.05).Conclusion The pulmonary alveolar lavage fluid Mycobacterium tuberculosis RNA combined with Xpert detection has an important clinical value in the diagnosis of smear-negative pulmonary tuberculosis and it is worthy of wide application.
