产前护理在改善产妇瘢痕子宫阴道分娩结局中的作用

摘要目的探讨产前护理在改善产妇瘢痕子宫阴道分娩结局中的影响。方法选取2017年1～8月江西省信丰县妇幼保健院收治的瘢痕子宫阴道分娩产妇60例作为研究对象，采用随机数字表法分为对照组和观察组，每组各30例。对照组采用常规护理模式，观察组采用产前综合护理干预模式。比较两组的焦虑、抑郁评分、护理满意评分、产妇分娩情况、产妇妊娠结局、2 h出血量、24 h出血量及并发症发生率。结果观察组的焦虑、抑郁评分低于对照组，护理满意评分高于对照组，自然分娩率高于对照组，孕妇胎儿生命体征正常率高于对照组，2 h出血量、24 h出血量少于对照组，并发症发生率低于对照组，差异均有统计学意义（P<0.05）。结论产前综合护理干预可有效改善瘢痕子宫产妇分娩前不良心理状况，缩短产程，降低剖宫产率，降低母婴的分娩风险，减少产妇产后出血量及并发症发生率，可满足患者对护理的需求，具有较高的临床推广价值。

Abstract：Objective To study the effect of prenatal care on improving the outcome of vaginal delivery in women with scarred uterus.Methods Sixty puerperae with scarred uterus via vaginal delivery in Maternal and Child Health Care Hospital of Xinfeng County from January to August 2017 were selected as the study subjects.By a random number table method,they were equally divided into control group and observation group.In the control group,routine nursing mode was used,while in the observation group,prenatal comprehensive nursing intervention was adopted.The scores of anxiety and depression,nursing satisfaction score,maternal delivery,maternal pregnancy outcome,2 h amount of bleeding,24 h amount of bleeding and the incidence of complications were compared.Results The anxiety and depression scores of the observation group were lower than those of the control group,the nursing satisfaction score was higher than that of the control group,the natural delivery rate was higher than that of the control group,the normal rate of vital signs in pregnant women and fetus was higher than that of the control group,the 2 h and 24 h amount of bleeding were both lesser than those in the control group,in the observation group,the incidence of complications was lower than that of the control group,the differences mentioned above all displayed statistical significance(P<0.05）.Conclusion Prenatal comprehensive nursing intervention can effectively improve the adverse psychological status of women with scar uterus before delivery,shorten the labor process,reduce the cesarean section rate,the risk of maternal delivery,and the postpartum hemorrhage and complications,which meets the patient′s care and has a high clinical promotion value.
