布地奈德联用孟鲁司特对儿童哮喘的临床效果分析

摘要目的探讨布地奈德联合孟鲁司特对儿童哮喘肺功能、免疫功能及生长发育的影响。方法选择2016年1月～2017年4月我院儿科收治的支气管哮喘患儿80例，按照随机数字表法分为观察组和对照组，每组各40例。对照组规律口服孟鲁司特钠咀嚼片5 mg/片，每晚睡前服用5 mg，观察组在对照组基础上联合吸入小剂量布地奈德鼻喷雾剂100 μg/喷，早晚各使用1次，两组均连续使用1年，比较两组干预后肺功能情况，干预后两组IgM、IgG及IgA水平，干预后CD4＋、CD8＋、CD4＋/CD8＋变化情况及干预后身高增长速度、体重增长速度及骨矿物含量。结果干预后，观察组 FEV1高于对照组（P＜0.05），FEV1/FVC 水平高于对照组（P＜0.05）；干预后，观察组 IgM、IgG 及 IgA 水平均显著高于对照组（P＜0.05），干预后观察组 CD4＋、CD8＋水平均高于对照组（P＜0.05），CD4＋/CD8＋比值大于对照组（P＜0.05）；干预后，观察组身高增长速度慢于对照组（P＜0.05），体重增长速度慢于对照组（P＜0.05），骨矿物含量低于对照组，差异均有统计学意义（P＜0.05）。结论对哮喘患儿，长时间吸入小剂量布地奈德能有效改善患儿肺功能，提高机体免疫能力，但对机体生长发育有一定能影响，故临床使用应注意权衡利弊。

Abstract：Objective To investigate the effect of Budesonide combined with Montelukast on pulmonary function,immune function and growth of children with asthma.Methods A total of 80 children with bronchial asthma admitted in our hospital from January 2016 to April 2017 were selected and were divided into observation group and control group,according to ramdom number table method,40 cases in each group.The control group was received regular oral administration of Montelukast,sodium chewable tablets 5 mg/tablet,taking 5 mg every night before going to bed,the observation group were inhaled a small dose of Budesonide nasal spray on the basis of the control group,the two groups were used continuously for 1 year.Then the lung function,the level of IgM,IgG and IgA,the changes of CD4+,CD8+,and CD4+/CD8+after intervention and the height growth rate,weight gain and bone mineral content after intervention were compared.ResultsAfter intervention,FEV1in the observation group was higher than that in the control group(P＜0.05）,and FEV1/FVC level in the observation group was higher than that in the control group(P＜0.05）.After intervention,IgM,IgG and IgA levels in the observation group were significantly higher than that in the control group(P＜0.05）,After intervention,the levels of CD4+and CD8+in the observation group were higher than that in the control group(P＜0.05）,and the ratio of CD4+/CD8+was greater than that in the control group(P＜0.05）.The height growth rate in the observation group was slower than that in the control group (P＜0.05）.The body weight growth rate in the observation group was slower than that in the control group (P＜0.05）,the bone mineral content in the observation group was lower than that in the control group,the differences were statistically significant(P<0.05）.Conclusion For children with asthma,inhaling a small dose of Budesonide for a long time can effectively improve the children′s lung function and body′s immune capacity,but it can have an impact on the body′s growth and development,so the clinical use should pay attention to the advantages and disadvantages.
