Influence of short-term insulin PumP intensive treatment on blood glucose control and vascular endothelial function in newly diagnosed tyPe 2 diabetic Patients

摘要目的观察短期胰岛素泵强化治疗对新诊断2型糖尿病患者血糖控制情况及对患者血管内皮功能的影响。方法选取我院2013年1月~2014年12月收治的新诊断2型糖尿病患者60例,将其随机分为观察组和对照组,各30例。观察组患者入院后给予胰岛素泵持续皮下输注,对照组给予胰岛素多次皮下注射,均连续治疗2周。观察两组治疗前后空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、空腹胰岛素(F-INS)、空腹C肽(F-C肽)及果糖胺(FA)情况,并应用高频彩色多普勒测量患者肱动脉血流介导血管舒张功能(FMD)。结果治疗后两组患者FBG、2 h PBG、FA均有所降低(P<0.05);观察组F-INS及F-C肽水平较治疗前有所升高,明显优于对照组的(P<0.05);观察组血糖达标时间短于对照组的,胰岛素用量少于对照组的,低血糖发生率低于对照组的(P<0.05);治疗前两组FMD比较,差异无统计学意义(P>0.05),治疗后两组FMD明显升高(P<0.05),观察组FMD明显优于对照组的(P<0.05)。结论新诊断2型糖尿病应用胰岛素泵短期强化治疗可以安全有效地控制血糖,改善患者血管内皮功能,与传统多次皮下注射胰岛素相比具有明显的优势。
Abstract:Objective To observe the influence of short-term insulin pump intensive treatment on blood glucose control and vascular endothelial function in newly diagnosed type 2 diabetic patients. Methods From January 2013 to December 2014,60 newly diagnosed type 2 diabetic patients treated in our hospital were evenly divided into observation group and control in random.In the observation group,continuous subcutaneous insulin pump infusion after being admitted into hospital was provided,while in the control group,subcutaneous injections of insulin for several times was adopted.They were treatmented two weeks.The fasting blood glucose(FBG),postprandial 2 h blood glucose(2 h PBG),fasting insulin(F-INS),fasting C-peptide(F-C peptide)and fructosamine(FA)before and after therapy were observed in both groups.Flow-mediated dilation(FMD)function of brachial artery was measured by high-frequency color Doppler. Results After therapy,FBG,2 h PBG,and FA in both groups were all decreased(P<0.05).In the observation group,the levels of F-INS and F-C peptide were increased to some extent compared with prior treatment and the increased levels were superior to those in the control group(P<0.05).In the observation group,the blood glucose reaching the standard was shorter than that of control group,dosage of insulin was fewer than that of control group,and incidence of hypoglycemia was lower than that of control group(P<0.05).There was no significant difference in FMD before treatment.After therapy,FMD in both groups was greatly improved,and improvement of FMD in the observation group was better than that of control group (P<0.05). Conclusion For newly diagnosed type 2 diabetic patients,application of insulin pump for short-term intensive treatment can control blood glucose safe and effective,and improve endothelial function,which has remarkable advantages in comparison with traditional subcutaneous injections of insulin for multiple times.
