Abstract：Anticoagulant therapy after heart valve replacement is of great clinical significance, and reasonable anticoagulant therapy is also of great significance for improving the long-term prognosis of patients with atrial fibrillation.The case recorded in this paper was a patient with atrial fibrillation after mechanical heart valve replacement, which has a higher risk of thromboembolism. Therefore, individualized anticoagulant therapy is particularly important. This article through the whole process of drug therapy for patients with monitoring, combining with the condition changes and clinical therapeutic effect of anticoagulant drugs, antibacterial and adjust heart failure treatment, eventually making treatment plan for patients, improve the practice ability of clinical pharmacists, and patients long-term effective and safe drug treatment is slow.
