腰椎牵引配合理疗治疗腰椎间盘突出症的效果

摘要目的观察腰椎牵引配合理疗治疗腰椎间盘突出症（LDH）的效果。方法选取2020年3～7月镇江瑞康医院收治的68 例LDH 患者作为研究对象，采用随机双盲法将其分为治疗组与对照组，每组各34 例。对照组患者仅接受腰椎牵引治疗，治疗组患者在对照组的基础上配合理疗治疗。观察两组患者的临床疗效、疼痛改善情况、直腿抬高角度及腰椎功能恢复情况。结果治疗后，治疗组与对照组患者的临床总有效率分别为94.12%、70.59%，治疗组的临床总有效率高于对照组，差异有统计学意义（P＜0.05）。治疗前，两组患者视觉模拟评分法（VAS）、感情疼痛分级指数（感情PRI）、感觉PRI 评分比较，差异均无统计学意义（P＞0.05）；治疗后，治疗组患者VAS、感情PRI、感觉PRI 评分均低于同组治疗前及对照组，差异有统计学意义（P＜0.05）。治疗前两组患者直腿抬高角度及Oswestry功能障碍指数（ODI）得分比较，差异均无统计学意义（P＞0.05）；治疗后治疗组患者直腿抬高角度高于同组治疗前及对照组，ODI 得分低于同组治疗前及对照组，差异有统计学意义（P＜0.05）。结论腰椎牵引配合理疗可显著提LDH 患者临床疗效，缓解其疼痛感，改善腰椎功能及下肢功能障碍，为临床治疗LDH 提供新的理论基础。

Abstract：Objective To observe the effect of lumbar traction combined with physical therapy in the treatment of lumbar disc herniation(LDH).Methods A total of 68 patients with LDH admitted to Zhenjiang Ruikang Hospital from March to July 2020 were enrolled as the research objects.They were divided into the treatment group and the control group by random double-blind method,with 34 cases in each group.Patients in the control group only received lumbar traction,and patients in the treatment group were treated with physical therapy on the basis of the control group.The clinical efficacy,pain improvement,straight leg elevation angle and lumbar function recovery of the two groups were observed.Results After treatment,the total clinical effective rate of the treatment group and the control group was 94.12% and 70.59%,respectively.The total clinical effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P＜0.05).Before treatment,there were no statistically significant differences in the visual analogue scale(VAS),emotional pain rating index(emotional PRI),and sensory PRI scores between the two groups(P＞0.05).After treatment,the scores of VAS,emotional PRI,and sensory PRI in the treatment group were lower than those of the same group before treatment and in the control group,and the differences were statistically significant(P＜0.05).Before treatment,there were no significant differences in the straight leg elevation angle and Oswestry disability index(ODI)score between the two groups(P＞0.05).After treatment,the straight leg elevation angle in the treatment group was higher than that of the same group before treatment and in the control group,and the ODI score was lower than that of the same group before treatment and in the control group,the differences were statistically significant(P＜0.05).Conclusion Lumbar traction combined with physical therapy can significantly improve the clinical efficacy of LDH patients,relieve their pain,improve lumbar spine function and lower limb dysfunction,and provide a new theoretical basis for clinical treatment of LDH.
