药敏检测指导在幽门螺杆菌根除治疗中的应用效果

摘要目的探讨药敏检测指导在幽门螺杆菌根除治疗中的应用效果。方法选取2017年1月～2019年1月在深圳市大鹏新区葵涌人民医院诊治的60例幽门螺杆菌感染患者作为研究对象，按照随机数字表法将入选患者分为观察组与对照组，各30例。两组患者均采用四联疗法进行治疗，对照组患者依据医生的临床经验给予枸橼酸铋钾胶囊、奥美拉唑肠溶胶囊、阿莫西林胶囊、克拉霉素进行治疗，观察组患者依据药敏检测结果选取两种抗生素联合奥美拉唑肠溶胶囊与枸橼酸铋钾胶囊进行治疗。记录观察组的药敏试验结果，比较两组治疗后的幽门螺杆菌根除率及不良反应总发生率，观察两组的治疗效果。结果药敏检测结果显示，阿莫西林的耐药率为0.00%，克拉霉素的耐药率为23.33%，甲硝唑的耐药率为80.00%，环丙沙星的耐药率为23.33%，利福平耐药率为3.33%。观察组的幽门螺杆菌根除率为93.33%，高于对照组的66.67%，差异有统计学意义（P＜0.05）。观察组的总有效率为96.67%，高于对照组的70.00%，差异有统计学意义（P＜0.05）。两组的不良反应总发生率比较，差异无统计学意义（P＞0.05）。结论药敏检测指导在幽门螺杆菌根除治疗中的应用效果显著，能够改善幽门螺杆菌根除率，值得临床推广应用。

Abstract：Objective To explore the application effect of drug sensitivity test guidance in the eradication therapy of Helicobacter pylori.Methods From January 2017 to January 2019,60 patients with Helicobacter pylori infection diagnosed and treated in Kuichong People′s Hospital of Dapeng New District of Shenzhen City were selected as the research objects,according to the method of random number table,the selected patients were divided into the observation group and the control group,30 cases in each group.Both groups were treated with quadruple therapy.The patients in the control group were treated with Bismuth Potassium Citrate Capsules,Omeprazole Enteric Coated Capsules,Amoxicillin Capsules and Clarithromycin according to the doctor′ clinical experience.Patients in the observation group were treated with two antibiotics combined with Omeprazole Enteric Coated Capsules and Bismuth Potassium Citrate Capsules according to the results of drug sensitivity test.The drug sensitivity test results of the observation group were recorded,and the eradication rate and the total incidence rate of adverse reaction of Helicobacter pylori in the two groups after treatment were compared,the therapeutic effect of the two groups was observed.Results Drug sensitivity test results showed that the drug resistances rates of Amoxicillin,Clarithromycin,Metronidazole,Ciprofloxacin and Rifampin were 0.00%,23.33%,80.00%,23.33% and 3.33%,respectively.The eradication rate of Helicobacter pylori in the observation group was 93.33%,which was higher than 66.67% in the control group,the difference was statistically significant (P＜0.05).The total effective rate of the observation group was 96.67%,which was higher than 70.00% of the control group,the difference was statistically significant (P＜0.05).There was no significant difference in the total incidence of adverse reactions between the two groups (P＞0.05).Conclusion The application and effect of drug sensitivity test guidance in the eradication therapy of Helicobacter pylori is significant,which can improve the eradication rate of Helicobacter pylori,and it is worthy of clinical promotion and application.
