Abstract:Objective To investigate the application effect of transitional care on fall prevention in stroke patients discharged from hospital.Methods A total of one hundred stroke patients treated in our hospital from January to October 2017 were selected in this study and they were divided into control group and observation group according to random number table method,50 cases in each group.The control group was given conventional discharge instructions,and followed up by telephone visit after one week.Besides routine discharge guidance,the observation group was established personal record and observation tables,followed up by closely transitional care and intervention.Two groups were assessed for functional recovery by Functional Independence Measure(FIM)table,and the incidence of falls within six months after discharge was compared.Results The score of FIM in the observation group was significantly higher than control group,and the fall incidence was significantly lower than control group,the differences were statistically significant(P<0.05).Conclusion Transitional care is beneficial to the recovery of daily activity of stroke patients,and can effectively prevent the incidence of falls after discharged from hospital.
