大剂量乌司他丁联合前列地尔对脓毒症患者炎性细胞的影响

摘要目的探讨大剂量乌司他丁联合前列地尔对脓毒症患者炎性细胞的影响。方法选取2016年6月～2017年10月我院收治的78例脓毒症患者作为研究对象，按随机数字表法分为对照组和观察组，每组各39例。对照组患者采取常规剂量乌司他丁联合前列地尔治疗，观察组患者采取大剂量乌司他丁联合前列地尔治疗，两组患者均治疗4周。观察比较两组患者治疗后肿瘤坏死因子-a（TNF-a）、白介素-6（IL-6）、白介素-8（IL-8）、白介素-10（IL-10）、白细胞（WBC）、C反应蛋白（CRP）、肌酐（Cr）、丙氨酸转移酶（ALT）等指标水平以及不良反应等情况。结果观察组患者治疗后的TNF-a、IL-6、IL-8水平均低于对照组，观察组患者治疗后的IL-10高于对照组，差异有统计学意义（P＜0.05）；观察组患者治疗后的 WBC、CRP、Cr、ALT 水平均低于对照组，差异有统计学意义（P＜0.05）；两组均未发生不良反应。结论脓毒症患者应用大剂量乌司他丁联合前列地尔治疗具有良好的临床效果，可显著改善脓毒症患者的临床症状，预防肝肾综合征，且安全性高，值得临床推广应用。

Abstract：Objective To investigate the effect of large dose ulinastatin combined with alprostadil on inflammatory cells in patients with pyemia.Methods A total of 78 patients with pyemia treated in our hospital from June 2016 to October 2017 were selected as the research object.According to the random number table method,the patients were divided into the control group and the observation group,with 39 cases in each group.The control group was treated with conventional dose ulinastatin combined with alprostadil,the observation group was treated with large dose ulinastatin combined with alprostadil,the two groups were treated for 4 weeks.The level of IL-6,IL-8,TNF-alpha,IL-10,WBC,CRP,Cr,ALT and other indicators,and adverse reactions and other specific circumstances were compared between the two groups after treatment.Results The levels of TNF-a,IL-6 and IL-8 in the observation group were lower than those in the control group,IL-10 in the observation group was higher than that in the control group after treatment,the difference was statistically significant(P＜0.05).The levels of WBC,CRP,Cr and ALT after treatment in the observation group were lower than those in the control group,the difference was statistically significant(P＜0.05),and no adverse reactions occurred in the two groups.Conclusion The treatment of pyemia with large dose of ulinastatin combined with prostadil has good clinical effect.It can significantly improve the clinical symptoms of sepsis,prevent hepatorenal syndrome,and has high safety.It is worthy of clinical application.
