Value analysis of patient-controlled analgesia by combined spinal-epidural anesthesia for labor analgesia

Objective To investigate the clinical value of patient-controlled analgesia by combined spinal-epidural anesthesia for labor analgesia.Methods From June to September 2017,80 cases of primipara with single infant and head delivery who were admitted to our hospital were selected as the subjects,and divided into the control group and analgesia group by random number table method,40 cases in each group.The control group was given routine treatment,without analgesic treatment.In the analgesia group,when cervix was open to 2 cm,epidural puncture was carried out at L3-4intervertebral space,and then subarachnoid puncture was carried out by placing lumbar puncture needle.One mg Bupivacaine was injected if successful.PCEA then injected with a mixture of 0.1%Ropivacaine and 2 μg/ml Ffentanyl.The analgesic effect,labor duration,mode of delivery,oxytocin use rate and neonatal scoring were observed in the two groups.Results The VAS scores in the analgesia group were significantly lower than those in the control group at 5 min,40 min,70 min and 100 min respectively,the differences were statistically significant(P<0.05).The duration of active period was shorter and the second stage of labor in the analgesia group was longer than those in the control group,all within the normal time range,and the difference was statistically significant(P<0.05).There was no significant difference in the third stage of labor(P>0.05).The use rate of oxytocin in the analgesia group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the mode of delivery between the two groups(P>0.05).Neonatal score between the two groups showed no statistically significant difference(P>0.05).Conclusion The combined spinal-epidural anesthesia for labor analgesia has an exact effect,which significantly reduces the VAS pain score of each stage of labor in puerpera,shortens the time of active period,extends the second stage of labor,has no impact on the third stage of labor,increases the use rate of oxytocin,it has no impact on the mode of delivery and neonatal score,which has a high clinical application value.
