益母草注射液联合卡前列素氨丁三醇预防剖宫产后出血的效果及对凝血的意义

摘要目的探讨益母草注射液联合卡前列素氨丁三醇对预防剖宫产后出血的临床效果和对凝血的意义。方法选取我院2009年10月～2015年12月收治的具有剖宫产手术指证的孕产妇196例，将其随机分为实验组和对照组。对照组98例，剖宫产术后于子宫肌壁注射卡前列素氨丁三醇；实验组98例，剖宫产术后子宫肌壁注射卡前列素氨丁三醇的基础上注射益母草注射液。比较两组孕妇术后2、24 h出血量，产后出血发生率，用药后宫缩持续时间，恶露持续时间及治疗前后孕妇的血浆纤维蛋白原（Fib）、D-二聚体（DD）、部分活化的凝血活酶时间（APTT）、凝血酶时间（TT）。结果实验组术后2、24 h出血量少于对照组，产后出血发生率低于对照组，用药后宫缩持续时间、恶露持续时间等短于对照组，差异有统计学意义（P＜0.05）；两组产妇血浆Fib、DD、APTT、TT等指标治疗后均较治疗前明显改善，差异有统计学意义（P＜0.05）；实验组治疗后的凝血指标较对照组治疗后显著降低，差异有统计学意义（P＜0.05）。结论益母草注射液联合卡前列素氨丁三醇预防剖宫产后出血疗效显著，值得临床推广。

Abstract：Objective To explore the clinical effect of Leonurus heterophyllus injection combined with carboprost tromethamine on preventing haemorrhage after cesarean section and its significance on coagulation.Methods From October 2009 to December 2015，196 pregnant and lying-in women with surgical indications of cesarean section admitted into our hospital were selected and evenly divided into experimental group and control group.In the control group，carboprost tromethamine was injected via wall of myometrium after cesarean section，while in the experimental group，on the basis of the control group，Leonurus heterophyllus injection was added.The amount of bleeding within 2 hours and 24 hours，incidence of postpartum hemorrhage，duration of uterine contraction after medication，duration of lochia，plasma fibrinogen （Fib），D-Dimer（DD），activated partial thromboplastin time（APTT），and thrombin time（TT）in pregnant women before and after therapy were compared.Results In the experimental group，the amount of bleeding within 2 hours and 24 hours was less than that of control group respectively，incidence of postpartum hemorrhage was lower than of control group，duration of uterine contraction after medication，and duration of lochia was greatly less than that in the control group respectively，with statistical differences（P＜0.05）.The plasma Fib，DD，APTT，TT after treatment in puerperae was greatly improved respectively in the two groups with statistical differences（P＜0.05）.In the experimental group，coagulation indicator after therapy was greatly decreased in comparison with that in the control group with a statistical difference　（P＜0.05）.Conclusion Leonurus heterophyllus injection combined with carboprost tromethamine on preventing haemorrhage after cesarean section is remarkable and is worthy of clinical promotion.
