医院-社区-家庭三元联动照护模式对脑卒中患者生活质量的影响

摘要目的探讨医院-社区-家庭三元联动照护模式对脑卒中患者生活质量的影响。方法抽取南昌市某三甲医院2015年6月～2016年6月出院的80例脑卒中患者作为研究对象，将其分为干预组和对照组，每组各40例。对照组患者采用常规出院护理模式，干预组患者采用三元联动护理模式。分别对两组患者出院时、出院后3个月及6个月的生活质量、神经功能缺损程度进行调查。结果干预组患者出院后3、6个月的NIHSS评分低于对照组，差异有统计学意义（P＜0.01）。干预组患者出院后3、6个月的生理、心理及独立性评分与对照组比较，差异有统计学意义（P＜0.05）。结论医院-社区-家庭三元联动照护模式可有效提高脑卒中患者生活质量及改善神经功能缺损情况。

Abstract：Objective To explore the effect of hospital-community-family ternary linkage nursing mode on quality of life of stroke patients.Methods 80 patients with stroke who were discharged from a grade A class three hospital in Nanchang from June 2015 to June 2016 were selected as study objects,and were divided into the intervention group and the control group,with 40 patients in each group.The control group was given a regular discharge nursing mode,and the intervention group was given a ternary linkage nursing mode.The quality of life and neurological defects were investigated in two groups of patients after discharge,3 months and 6 months after discharge.Results The NIHSS score of the intervention group was lower than the control group for 3 months and 6 months after discharge,the difference was statistically significant(P＜0.01).The physiological,psychological and independent scores of the patients for 3 months and 6 months after discharge of the intervention group were compared with control group,and the differences were statistically significant (P＜0.05).Conclusion The hospital-community-family ternary linkage nursing model can effectively improve the quality of life of stroke patients and improve the neurological deficits.
