呼伦贝尔地区中老年人血清25-羟维生素D3水平与代谢综合征的关系研究

摘要目的探讨呼伦贝尔地区中老年人血清25-羟维生素D3[25（OH）D3]水平与代谢综合征（MS）的关系。方法选取2017年1月—2019年8月于内蒙古呼伦贝尔市人民医院就诊的2145 例MS 患者作为观察组，选取同期于内蒙古呼伦贝尔市人民医院就诊的684 例非MS 患者作为对照组。比较两组的体重指数（BMI）、收缩压、舒张压、血生化指标[25（OH）D3、总胆固醇（TC）、三酰甘油（TG）、低密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL-C)、空腹血糖（FBG）、尿酸（UA）、糖化血红蛋白（HbAlc）]，分析观察组25（OH）D3 与BMI、收缩压、舒张压、血生化指标的相关性，并比较两组的25（OH）D3 水平。结果观察组的BMI、LDL-C、TC、TG、HbA1c、FBG、收缩压、舒张压高于对照组，25（OH）D3 低于对照组，差异有统计学意义（P＜0.05）；两组的HDL-C、UA 比较，差异无统计学意义（P＞0.05）。观察组的25（OH）D3 水平与BMI、HbA1c、FBG、舒张压、LDL-C、收缩压呈负相关（P＜0.05）。观察组的25（OH）D3≤20 ng/mL、＞20～≤30 ng/mL 比例高于对照组，25（OH）D3＞30 ng/mL 比例低于对照组，差异有统计学意义（P＜0.05）。结论呼伦贝尔地区中老年MS 患者血清25（OH）D3 水平相对不足，其水平高低与BMI、HbA1c、FBG、舒张压、LDL-C、收缩压密切相关，25（OH）D3 水平降低可能与MS 的发生有关。

Abstract：Objective To explore the relationship between serum 25-hydroxyvitamin D3[25(OH)D3]levels and metabolic syndrome(MS)in the elderly in Hulunbuir area.Methods A total of 2145 MS patients who were admitted to the Hulunbuir People′s Hospital of Inner Mongolia from January 2017 to August 2019 were selected as the observation group,and 684 non-MS patients treated in Hulunbuir People′s Hospital of Inner Mongolia during the same period were selected as the control group.The body mass index(BMI),systolic blood pressure,diastolic blood pressure,blood biochemical indexes[25(OH)D3,total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),fasting blood glucose(FBG),uric acid(UA),and glycated hemoglobin(HbA1c)]were compared between the two groups,the correlation between 25(OH)D3 in the observation group and BMI,systolic blood pressure,diastolic blood pressure and blood biochemical indexes was analyzed,and the 25(OH)D3 levels in the two groups were compared.Results The BMI,LDL-C,TC,TG,HbA1c,FBG,systolic blood pressure and diastolic blood pressure of in observation group were higher than those in control group,and 25(OH)D3 was lower than that in the control group,the differences were statistically significant(P<0.05); There were no significant differences in HDL-C and UA between the two groups(P>0.05).The 25(OH)D3 level in the observation group was negatively correlated with BMI,HbA1c,FBG,diastolic blood pressure,LDL-C and systolic blood pressure(P＜0.05).The proportions of 25(OH)D3≤20 ng/mL and >20～30 ng/mL in observation group were higher than those in control group,and the proportions of 25(OH)D3> 30 ng/mL were lower than those in control group,the differences were statistically significant(P<0.05).Conclusion The serum 25(OH)D3 level is relatively insufficient in the middle-aged and elderly MS patients in Hulunbuir area,and its level is closely related to BMI,HbAlc,FBG,diastolic blood pressure,LDL-C,and systolic blood pressure.The decrease of 25(OH)D3 level may be related to MS Happened related.
