全程责任制护理模式在剖宫产产妇宫缩乏力性产后出血护理中的应用价值

摘要目的分析全程责任制护理模式在剖宫产宫缩乏力性产后出血护理中的应用价值，以控制产妇宫缩乏力性产后出血风险，改善产妇预后。方法回顾性分析2018年1月～2019年5月瑞金市妇幼保健院收治的60 例剖宫产宫缩乏力性产后出血产妇的临床资料，依据护理方法的不同分为对照组（n=33）与观察组（n=27）。对照组应用常规护理，观察组应用全程责任制护理模式干预。比较两组的产后出血量及焦虑、抑郁情况。结果两组产后24 h的总出血量高于产后2 h，观察组产后2、24 h 内的总出血量低于对照组，差异均有统计学意义（P＜0.05）。干预前，两组的焦虑自评量表（SAS）与抑郁自评量表（SDS）评分比较，差异无统计学意义（P＞0.05）；干预5 d 后，两组的SAS 与SDS 评分低于干预前，且观察组的SAS 与SDS 评分低于对照组，差异均有统计学意义（P＜0.05）。结论全程责任制护理模式在剖宫产宫缩乏力性产后出血护理的应用效果较好，可有效控制产后的总出血量，改善产妇焦虑、抑郁情绪，对产后恢复有积极意义。

Abstract：Objective To analyze the application value of the whole-course responsibility nursing model in the nursing of parturient with postpartum hemorrhage caused by uterine inertia and cesarean section.Methods The clinical data of 60 parturient with postpartum hemorrhage caused by uterine inertia and cesarean section treated in Ruijin Maternal and Child Health Hospital from January 2018 to May 2019 was retrospectively analyzed,they were divided into the control group(n=33)and the observation group(n=27)by the nursing methods difference.The control group was used routine nursing,the observation group was used whole course responsibility nursing mode.The amount of postpartum hemorrhage,anxiety and depression were compared between the two groups.Results The total amount of bleeding 24 hours after delivery was higher than that 2 hours after delivery,the total amount of bleeding within 2 hours and 24 hours after delivery in the observation group were lower than those in the control group,and the differences were statistically significant(P＜0.05).Before intervention,the scores of the self-rating anxiety scale(SAS)and self-rating depression scale(SDS)of the two groups,there were no statistical significant differences(P＞0.05);after 5 days of intervention,the SAS and SDS scores of the two groups were lower than those before intervention,and the SAS and SDS scores of the observation group were lower than those of the control group,the differences were statistically significant(P ＜0.05).Conclusion The full-course responsibility nursing model has a good application effect in the nursing of parturient with postpartum hemorrhage caused by uterine inertia and cesarean section.It can effectively control the amount of postpartum hemorrhage of the parturient,improve the anxiety and depression of the parturient,and has a positive significance for postpartum recovery.
