Correlation analysis of red blood distribution width levels with blood pressure variability in patients with prim-ary hypertension

摘要目的 分析红细胞分布宽度(RDW)在原发性高血压患者与血压变异性(BPV)的相关性。方法 回顾性分析2017年10月~2019年6月北京市西城区平安医院收治的220例原发性高血压患者的临床资料。监测所有受试者24 h 动态血压,测定血清RDW 和血脂等生化指标。根据RDW 四分位值将患者分为4组:Ⅰ组58例(RDW<12.40%)、Ⅱ组56例(12.40%≤RDW<12.90%)、Ⅲ组52例(12.90%≤RDW<13.50%)、Ⅳ组54例(RDW≥13.50%)。本研究以24 h 动态血压读数的标准差作为BPV指标[包括24 h 收缩压标准差(24 h SSD)、24 h 舒张压标准差(24 h DSD)],比较不同RDW 水平患者的BPV 情况,分析RDW 与BPV的相关性。结果 BPV指标比较结果显示,24 h SSD、24 h DSD 在Ⅳ组高于Ⅲ组、Ⅲ组高于Ⅱ组、Ⅱ组高于Ⅰ组,差异有统计学意义(P<0.05)。Pearson 相关性分析结果显示,RDW 与24 h SSD、24 h DSD 呈正相关(r=0.727、0.557,P<0.05)。结论 原发性高血压患者RDW 水平与BPV 呈正相关,RDW 与原发性高血压患者BPV 密切相关。
Abstract:Objective To analyze the correlation of red blood distribution width (RDW) levels with blood pressure variability (BPV) in patients with primary hypertension.Methods Retrospective analysis was made on the clinical data of 220 patients with essential in Beijing Xicheng District Pingan Hospital from October 2017 to June 2019.24 hours ambulatory blood pressure monitoring was conducted.Plasma levels of RDW, fasting blood glucose and lipids were also examined.Patients were divided into 4 groups based on their RDW levels: group Ⅰwith 58 cases (RDW<12.4%),group Ⅱwith 56 cases (12.4%≤RDW<12.9%), group Ⅲwith 52 cases (12.9%≤RDW<13.5%), group Ⅳwith 54 cases (RDW≥13.5%).In this study, the standard deviation of 2-hour ambulatory blood pressure reading was used as the index of BPV, including 24 hours systolic blood pressure standard deviation (24 h SSD), 24 hours diastolic blood pressure standard deviation (24 h DSD).The BPV of patients with different RDW levels was compared and the correlation between RDW and BPV was analyzed.Results Comparison of BPV indicators showed that the levels of 24 h SSD and 24 h DSD of group Ⅳwere higher than those of the group Ⅲ, the levels of 24 h SSD and 24 h DSD of groupⅢwere higher than those of the group Ⅱ, and the levels of 24 h SSD and 24 h DSD of group Ⅱwere higher than those of the group Ⅰ, and the differences were statistically significant (P<0.05).The result of Pearson correlation analysis showed that RDW was positively correlated with 24 hours SSD and 24 hours DSD (r=0.727 and 0.557, P<0.05).Conclusion The RDW level of patients with essential hypertension is positively correlated with BPV, RDW is closely related to BPV in patients with essential hypertension.
