纤维支气管镜肺泡灌洗联合美罗培南治疗重症肺部感染的效果

摘要目的探讨纤维支气管镜肺泡灌洗联合美罗培南治疗重症肺部感染的效果。方法选取2017年2月～2019年5月重庆市开州区人民医院诊治的80 例重症肺部感染患者作为研究对象。按照随机数字法分为对照组（40 例）和实验组（40 例），对照组采取美罗培南治疗，实验组采取纤维支气管镜肺泡灌洗联合美罗培南治疗。比较两组患者的临床疗效、肺功能指标、临床症状恢复时间以及炎性指标。结果实验组总有效率高于对照组，差异有统计学意义（P<0.05）；实验组最大通气量（MMV）、最大呼气中期流量（MMEF）、第1 秒钟用力呼吸容积/用力肺活量（FEV1/FVC）高于对照组，差异有统计学意义（P<0.05）；实验组体温恢复时间、咳嗽恢复时间短于对照组，差异有统计学意义（P<0.05）；两组患者治疗前白细胞计数（WBC）、降钙素原（PCT）比较，差异无统计学意义（P＞0.05）；两组患者治疗后WBC、PCT 低于治疗前，差异有统计学意义（P<0.05）；实验组治疗后WBC、PCT 低于对照组，差异有统计学意义（P<0.05）。结论纤维支气管镜肺泡灌洗联合美罗培南治疗重症肺部感染效果确切，可有效改善患者的肺功能和炎性指标，缩短治疗时间，因此值得推广应用。

Abstract： Objective To investigate the effect of fiberoptic bronchoscopic alveolar lavage combined with Meropenem in the treatment of severe pulmonary infection. Methods A total of 80 patients with severe pulmonary infection admitted to People′s Hospital of Kaizhou District in Chongqing City from February 2017 to May 2019 were selected as the research objects. They were divided into the control group (40 cases) and the experimental group (40 cases) according to the random number method. The control group was treated with Meropenem, the experimental group took fibrobronchoscope alveolar lavage combined with Meropenem treatment, and the clinical efficacy, lung function indexes,clinical symptom recovery time and inflammatory indexes of patients of the two groups were compared. Results The total effective rate of the experimental group was higher than that of the control group, the difference was statistically significant (P<0.05). The maximal ventilatory volume (MMV), maximal middle expiratory flow (MMEF), and 1st second forced breathing volume/forced vital capacity (FEV1/FVC) in the experimental group were higher than those of the control group, the differences were statistically significant (P<0.05). The recovery time of body temperature and cough recovery time in experimental group were shorter than those in control group, the differences were statistically significant (P<0.05). There were no significant differences in white blood cell count (WBC) and procalcitonin (PCT)between the two groups before treatment (P>0.05). The WBC and PCT of the two groups after treament were lower than those before teatment, the differences were statistically significan (P<0.05). WBC and PCT levels in the experimental group after treatment were lower than those in the control group, the differences were statistically significant (P<0.05).Conclusion Fiberoptic bronchoscopic alveolar lavage combined with Meropenem has a definite curative effect on severe pulmonary infection, effectively improves the patient′s lung function and inflammatory index, and shortens the treatment time, so it is worth popularizing and applying.
