Clinical application of bedside bronchoscopy sputum suction and alveolar lavage in the treatment of patients with acute massive cerebral infarction and stroke associated pneumonia

Abstract:Objective To investigate the clinical application of bedside bronchoscopy sputum suction and alveolar lavage in the treatment of patients with acute massive cerebral infarction and stroke associated pneumonia.Methods A total of 100 patients with acute massive cerebral infarction hospitalized in neurology department of Guangdong Tongjiang hospital from January 2020 to December 2020 were selected as the research objects.They were divided into control group and treatment group according to the block grouping method,with 50 cases in each group.The control group was treated with routine treatment,nutritional support,professional nursing and routine antibiotics.On the basis of the treatment of the control group,bedside bronchoscopy was established as soon as possible after the diagnosis of stroke-associated pneumonia,and bronchoscopic sputum aspiration combined with alveolar lavage was adopted in the treatment group.The changes of white blood cell count(WBC),C-reactive protein(CRP),interleukin-6(IL-6),procalcitonin(PCT),oxygenation index,length of hospital stay and rate of endotracheal intubation were compared between the two groups.Results After one week of treatment,the WBC,CRP,IL-6 and PCT of the treatment group were lower than those of the control group,and the oxygenation index was higher than that of the control group,the differences were statistically significant(P<0.05).The length of hospitalization in the treatment group was shorter than that of the control group,and the rate of endotracheal intubation was lower than that of the control group,the differences were statistically significant(P<0.05).Conclusion The clinical application of bedside bronchoscopy sputum suction and alveolar lavage in the treatment of patients with acute massive cerebral infarction and stroke associated pneumonia can control infection early,improve arterial blood gas index and inflammatory index,shorten hospitalization time and reduce the rate of tracheal intubation.
