Effect of alveolar lavage on blood gas analysis and serum inflammatory indexes in emergency patients with severe pneumonia with respiratory failure

Abstract:Objective To investigate the effect of alveolar lavage on blood gas analysis and serum inflammatory indexes in patients with severe pneumonia complicated with respiratory failure.Methods A total of 68 patients with severe pneumonia complicated with respiratory failure who were treated in the Emergency Department of Nanchang Hongdu Hospital of Traditional Chinese Medicine from January 1,2017 to January 1,2020 were selected and divided into the observation group(35 cases)and the control group(33 cases)according to the random number table method.The control group was given conventional treatment,and the observation group was given bronchoalveolar lavage on the basis of conventional treatment.After 3 days of treatment,the clinical efficacy,severity of the disease,blood gas analysis and serum inflammatory indexes of the two groups were compared.Results After treatment,the total effective rate of the observation group(97.14%)was higher than that of the control group(66.67%),and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in acute physiology and chronic health(APACHEⅡ)score between the two groups(P>0.05).After treatment,APACHEⅡscores of the two groups were lower than those before treatment,and the differences were statistically significant(P<0.05).After treatment,APACHEⅡscore of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no statistically significant differences in arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2)between the two groups(P>0.05).After treatment,the PaO2 levels of the two groups were higher than those before treatment,the PaCO2 levels of the two groups were lower than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the PaO2 of the observation group was higher than that of the control group,PaCO2 of the observation group was lower than that of the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no statistically significant differences in the levels of serum C-reactive protein(CRP),procalcitonin(PCT)between the two groups(P>0.05).After treatment,the levels of CRP and PCT in the two groups were lower than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the levels of CRP and PCT in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The curative effect of bronchoalveolar lavage in emergency patients with severe pneumonia complicated with respiratory failure is exact,which can effectively reduce the symptoms of respiratory failure,inhibit the inflammatory reaction of the body and significantly alleviate the condition,which has high clinical application value.
