透明帽辅助结肠镜下聚桂醇不同给药方式硬化疗法治疗Ⅰ、Ⅱ期出血性内痔的临床效果

摘要目的评价透明帽辅助结肠镜下聚桂醇不同给药方式硬化疗法治疗Ⅰ、Ⅱ期出血性的效果及安全性。方法选取2019年1月～2020年1月于吉林省一汽总医院消化内科就诊的130例Ⅰ、Ⅱ期出血性内痔患者作为研究对象。按照入院时间前后顺序将患者分为对照组（75例）和观察组（55例）。对照组患者依照疾病分期分为对照1组（Ⅰ期，42例）和对照2组（Ⅱ期，33例）。观察组患者依照疾病分期分为观察1组（Ⅰ期，32例）和观察2组（Ⅱ期，23例）。对照1组及对照2组均采用透明帽辅助结肠镜下聚桂醇液体硬化剂处理方式进行硬化疗法，观察1组及观察2组采用透明帽辅助结肠镜下聚桂醇泡沫硬化剂处理方式进行硬化疗法。比较各组患者的聚桂醇总用量、总有效率及并发症发生情况。结果观察1组的聚桂醇总用量与观察2组比较，差异无统计学意义（P＞0.05）；观察1组的聚桂醇总用量少于对照1组，差异有统计学意义（P＜0.05）；观察2组的聚桂醇总用量少于对照2组，差异有统计学意义（P＜0.05）。观察1组与对照1组的治疗总有效率比较，差异无统计学意义（P＞0.05）；观察2组与对照2组的治疗总有效率比较，差异无统计学意义（P＞0.05）；观察1组的术后并发症总发生率低于对照1组，差异有统计学意义（P＜0.05）；观察2组的术后并发症总发生率低于对照2组，差异有统计学意义（P＜0.05）。结论透明帽辅助结肠镜下，聚桂醇硬化剂注射治疗Ⅰ、Ⅱ期出血性内痔是安全、有效的。其中聚桂醇泡沫给药方式与液体给药比较，给药量更少，并发症发生率更低，具有明显优势。

Abstract：Objective To evaluate the efficacy and safety of sclerotherapy with different administration methods of lauromacrogol under transparent cap assisted colonoscopy in the treatment of stage I andⅡhemorrhagic internal hemorrhoids.Methods A total of 130 patients with stageⅠandⅡhemorrhagic internal hemorrhoids were selected as subjects from January 2019 to January 2020 in the Department of Gastroenterology in the General Hospital of China First Automobile Works Group,Jilin Province.The patients were divided into control group(75 cases)and observation group(55 cases)according to the order of admission time.The patients in the control group were divided into control group 1(stageⅠ,42 cases)and control group 2(stageⅡ,33 cases).The patients in the observation group were divided into observation group 1(stageⅠ,32 cases)and observation group 2(stageⅡ,23 cases).The control group 1 and control group 2 were treated with lauryl alcohol liquid sclerotherapy under colonoscopy assisted by transparent cap.The observation group 1 and the observation group 2 were treated with lauromacrogol foam sclerotherapy under colonoscopy assisted by transparent cap.The total dosage of lauryl alcohol,the total effective rate and the incidence of complications were compared.Results There was no significant difference in the total amount of lauromacrogol between the observation group 1 and the observation group 2(P＞0.05).The total amount of lauromacrogol in observation group 1 was less than control group 1,and the difference was statistically significant(P＜0.05).The total amount of lauromacrogol in observation group 2 was less than control group 2,and the difference was statistically significant(P＜0.05).There was no significant difference in the total effective rate between the observation group 1 and the control group 1(P＞0.05).There was no significant difference in the total effective rate between the observation group and the control group(P＞0.05).The total incidence of postoperative complications in observation group 1 was lower than control group 1,and the difference was statistically significant(P＜0.05).The total incidence of postoperative complications in observation group 2 was lower than control group 2,and the difference was statistically significant(P＜0.05).Conclusion Under colonoscopy assisted by transparent cap,the injection of lauromacrogol sclerosis agent is safe and effective in the treatment of stage I andⅡhemorrhagic internal hemorrhoids.Compared with liquid administration,foam administration of lauromacrogol has less dose and lower complication rate,which has obvious advantages.
