中医情志护理对行无痛胃镜检查患者麻醉效果及心理性疼痛的研究

摘要目的探讨中医情志护理在无痛胃镜检查患者中的应用效果。方法选择2018年10月至2020年10月于九江市中医医院无痛胃镜检查70 例患者，按随机数字表法分为对照组和观察组，每组各35 例。对照组采取常规干预，观察组在此基础上实施中医情志护理。比较两组检查前后的心理状态[焦虑自评量表（SAS）、抑郁自评量表（SDS）]和心理性疼痛[六点行为评分法（BRS-6）]评分，统计并比较两组的麻醉效果，调查患者对护理满意度。结果观察组检查后的SAS 为（22.35±3.11）、SDS 为（23.50±3.14）、BRS-6 为（1.37±0.13）分，低于对照组的（28.98±3.15）、（29.91±3.28）、（2.20±0.16）分，睫毛反射消失时间为（40.53±4.32）s、清醒时间为（6.02±0.08）min，短于对照组的（55.84±5.21）s、（6.45±0.12）min，丙泊酚用量为（2.71±0.19）mg/kg，少于对照组的（4.59±0.52）mg/kg，护理总满意度为94.29%，高于对照组的77.14%，差异均有统计学意义（P＜0.05）。结论中医情志护理能改善无痛胃镜检查患者消极情绪，缓解患者心理性疼痛，提升麻醉效果，提高患者满意度。

Abstract：Objective To explore the effect of traditional Chinese medicine(TCM)emotional nursing in patients with painless gastroscopy.Methods A total of 70 patients underwent painless gastroscopy in iujiang Hospital of Traditional Chinese Medicine from October 2018 to October 2020 were selected and divided into the control group and the observation group by random number table method,35 patients in each group.Patients in the control group were given routine intervention,while patients in the observation group were given TCM emotional nursing on this basis.Psychological state including self-rating anxiety scale(SAS),self-rating depression scale(SDS)and psychological pain including the 6-point behavioral rating scale(BRS-6)scores were compared between the two groups before and after examination.Anesthetic effect was statistically compared between the two groups,and patients′satisfaction with nursing was investigated.Results After examination,the scores of SAS was(22.35±3.11),SDS was(23.50±3.14)and BRS-6 was(1.37±0.13)points in the observation group，lower than those in the control group of(28.98±3.15),(29.91±3.28)and(2.20±0.16)points,respectively.The disappearance time of eyelash reflex was(40.53±4.32)s and waking time was(6.02±0.08)min in the observation group，shorter than those of the control group of(55.84±5.21)s and(6.45±0.12)min,respectively.The dosage of propofol was(2.71±0.19)mg/kg in the observation group,less than the control group of(4.59±0.52)mg/kg,the nursing total satisfaction of the observation group was 94.29%,higher than that of the control group accounting for 77.14%,the differences were statistically significant(P＜0.05).Conclusion TCM emotional nursing can improve the negative emotions of patients undergoing painless gastroscopy,relieve the psychological pain of patients,enhance anesthetic effect,and increase patients′ satisfaction.
