危机式护理管理理论的实施对产妇分娩结局的影响

摘要目的探讨危机式护理管理理论的实施对产妇分娩结局的影响。方法选取2018年5月—2020年5月于大连市妇女儿童医疗中心分娩的108 例产妇作为研究对象，按照随机数字表法将其分为对照组与观察组，每组各54 例。对照组采取常规护理干预，观察组实施危机式护理管理理论的护理干预。比较两组的产程、分娩方式、产后并发症、不良新生儿结局发生情况及产妇对护理的满意度。结果观察组的第一产程、第二产程、第三产程均短于对照组，差异有统计学意义（P＜0.05）。观察组的自然分娩率高于对照组，剖宫产率、阴道助产率均低于对照组，两组分娩方式比较，差异有统计学意义。产后并发症总发生率和不良新生儿结局总发生率均低于对照组，差异有统计学意义（P＜0.05）。观察组产妇的护理满意度评分高于对照组，差异有统计学意义（P＜0.05）。结论产妇分娩过程中采用危机式护理管理理论干预，能够缩短产程，改善分娩结局，减少并发症发生，从而提高产妇满意度。

Abstract：Objective To explore the effect of implementing crisis nursing management theory on the delivery outcomes in puerperae.Methods A total of 108 women who delivered in Dalian Women and Children′s Medical Center from May 2018 to May 2020 were selected as the research objects.According to random number table method, they were divided into the control group and the observation group, with 54 cases in each group.The control group received routine nursing intervention, and the observation group received nursing intervention with crisis nursing management theory.The stage of labor, mode of delivery, postpartum complications, occurrence of adverse neonatal outcomes and maternal satisfaction with nursing were compared between the two groups.Results The first, second and third stages of labor in the observation group were shorter than those in the control group, and the differences were statistically significant (P＜0.05).The rate of natural delivery in the observation group was higher than that in the control group, the rate of cesarean section and vaginal delivery were lower than those in the control group, the difference of delivery mode between the two groups was statistically significant.The total incidence of postpartum complications and the total incidence of adverse neonatal outcomes were lower than those in the control group, and the differences were statistically significant (P＜0.05).The nursing satisfaction score of the observation group was higher than that of the control group, and the difference was statistically significant (P＜0.05).Conclusion The intervention of crisis nursing management theory in the process of childbirth can shorten the labor stages, improve the delivery outcome, and reduce the incidence of complications, so as to improve the satisfaction of the puerperae.
