输卵管三维可视化在不孕症微创术中的应用效果

摘要目的探讨输卵管三维可视化在不孕症微创术中的应用效果。方法选取2017年8月～2018年8月萍乡市人民医院收治的42 例输卵管堵塞不孕患者作为研究对象，按照随机数字表法将其分为研究组（21 例）与对照组（21例）。对照组采用传统不孕症宫腔镜联合腹腔镜微创术治疗，研究组在输卵管三维可视化辅助下进行不孕症宫腹腔镜联合微创术治疗。比较两组的手术时间、术中出血量、阴道停止出血时间、术后3 个月输卵管复发阻塞率、术后3 个月内并发症总发生率、术后1年内两组妊娠率及术后2年内成功分娩率。结果研究组的手术时间、阴道停止出血时间短于对照组，术后出血量少于对照组，术后3 个月内并发症总发生率低于对照组，差异均有统计学意义（P＜0.05）。术后3 个月，研究组输卵管复发阻塞率低于对照组，差异有统计学意义（P＜0.05）。研究组术后1年内妊娠率、术后2年内成功分娩率高于对照组，差异有统计学意义（P＜0.05）。结论采用于不孕症微创术中应用输卵管三维可视化技术可明显提高手术疗效，改善患者生殖功能，适宜临床推广应用。

Abstract：Objective To investigate the effect of three-dimensional visualization of fallopian tube in minimally invasive surgery for infertility.Methods A total of 42 infertile women with fallopian tube block treated in Jiangxi Pingxiang People′s Hospital from August 2017 to August 2018 were selected as the research subjects.According to the random number table method,they were divided into study group(21 cases)and control group(21 cases).The control group was treated with traditional hysteroscopy for infertility combined with laparoscopic minimally invasive surgery,and the study group was given hysteroscopy for infertility combined with minimally invasive surgery with the aid of three-dimensional visualization of fallopian tube.The surgical time,intraoperative blood loss,time of vaginal bleeding stop and recurrence rate of fallopian tube block at 3 months after surgery,total incidence rate of complications within 3 months after surgery,pregnancy rate within 1 year after surgery and delivery success rate within 2 years after surgery were compared between the two groups.Results The intraoperative blood loss and total incidence rate of complications within 3 months after surgery in the study group were lower than those in the control group,the surgical time and vaginal bleedin stop time in the study group was chorter than those in the control group,with statistical significance(P＜0.05).The recurrence rate of fallopian tube block at 3 months after surgery in study group was lower than that in control group,the difference was statistically significant(P＜0.05).The pregnancy rate in 1 year and successful delivery rate in 2 years in the study group were higher than those in the control group,and the differences were statistically significant(P＜0.05).Conclusion The application of tubal three-dimensional visualization technology in infertility microtrauma can significantly improve the surgical effect,improve the reproductive function of patients which is suitable for clinical application.
