Abstract:Objective To investigate the clinical effect of early hyperbaric oxygen(HBO)therapy for wake-up promotion in comatose patients with severe craniocerebral injury.Methods A total of 120 cases of comatose patients with severe craniocerebral injury admitted to the First Affiliated Hospital of Nanchang University from January 2015 to December 2017 were selected as the research objects,and they were divided into the treatment group(n=60)and the control group(n=60)according to different treatment methods.The control group received traditional rehabilitation treatment for wake-up promotion,and on this basis,the treatment group was given HBO treatment(group A[n=10]:course≤10 days,HBO treatment 1-10 times;group B[n=10]:course≤10 days,HBO treatment 11-20 times;group C[n=10]:course≤10 days,HBO treatment 21-30 times;group D[n=10]:course 11-20 days,HBO treatment 1-10 times;group E[n=10]:course 11-20 days,HBO treatment 11-20 times;group F[n=10]:course 11-20 days,HBO treatment 21-30 times).The Glasgow coma scale(GCS)scores were compared in each group before treatment,at 10,20 and 30 days after treatment.Results There were no statistically significant differences in the GSC scores of each group before treatment and at 10 days after treatment(P>0.05).The GSC scores of the experimental group were(9.62±2.68)and(12.48±2.01)points at 20 and 30 days after treatment,which were higher than the control group′s(7.39±2.14)and(8.05±1.45)points,the differences were statistically significant(P<0.05).Among the patients with disease course≤10 d in the experimental group,the GSC scores in group C were(13.45±2.32)and(15.20±2.15)points at 20 and 30 days after treatment,which were higher than those in group B of(11.01±2.01)and(12.05±2.01)points,also higher than those in group A of(8.95±1.98)and(10.56±1.95)points,the differences were statistically significant(P<0.05).Among patients with disease course from 11 to 20 days,the GSC scores of group F were(11.32±1.95)and(12.48±2.37)points at 20 and 30 days after treatment,which were higher than those of group E for(9.01±2.29)and(10.56±1.65)points,also higher than those of group D for(7.43±2.05)and(8.12±1.36)points,the differences were statistically significant(P<0.05).For the same number of treatments in the experimental group,at 20 and 30 days after treatment,the GSC score of group A was higher than that of group D,the GSC score of group B was higher than that of group E,and the GSC score of group C was higher than that of group F,the differences were statistically significant(P<0.05).Conclusion The wake-up promotion effect of early HBO in the treatment of comatose patients with severe craniocerebral injury is better,and the best time window for HBO treatment is within 10 days after the onset of the disease,and 21-30 times of treatment is appropriate.
