Effect com parison of nasal high-flow oxygen therapy and non-invasive positive pressure ventilation in the alternate treatment of acute exacerbation of chronic obstructive pulmonary disease with respiratory failure

Abstract:Objective To investigate the clinical effect of nasal high-flow oxygen therapy and non-invasive positive pressure ventilation (NIPPV)in the alternate treatment of acute exacerbation of chronic obstructive pulmonary disease(COPD)with respiratory failure.Methods From March 2018 to September 2019,80 patientswith acute exacerbation of COPD with respiratory failure who were treated in emergency intensive care unit of our hospital were selected as the research subjects.They were divided into two groups according to the random number tablemethod,each with 40 patients.In the control group,NIPPV was use,and in the observation group,alternate nasal high-flow oxygen therapy with NIPPV was applied.The lung function indicators 1week after treatment of the two groups were compared.The St George′s respiratory questionnaire(SGRQ)scores before and after treatment of the two groupswere compared.The time of using mechanical ventilator and hospital stay were compared between the two groups.The transfer to the general ward after improvement of the two groupswas also counted.Results Oneweek after treatment,the level of forced expiratory volume in 1second (FEV1)and the ratio of FEV1 to forced vital capacity (FEV1/FVC)in the observation group were higher than those in the control group (P<0.05).The SGRQ scores at 1d after treatment and 1week after treatment in both groupswere higher than those before the treatment(P<0.05).The SGRQ scores at 1d after treatment and 1week after treatment of the observation group were higher than those of the control group(P<0.05).In the observation group,the time of usingmechanical ventilator and hospital stay were shorter than those in the control group (P<0.05).The proportion of patients in the observation group who were transferred to generalwards after improvementwas higher than that in the control group(P<0.05).Conclusion For patientswith acute exacerbation of COPD combined with respiratory failure,the implementation of alternate nasal high-flow oxygen therapy and NIPPV can effectively improve patient′s lung function,ameliorate quality of life,shorten hospital stay,and enhance the therapeutic effect.
