Application effect ultrasound-guided transabdominal transverse muscle plane nerve block combined with intravenous analgesia pump in the treatment of cholangiocarcinoma after radical operation

Abstract:Objective To explore the application effect of ultrasound-guided transabdominal transverse muscle plane nerve block combined with intravenous analgesia pump in the treatment of cholangiocarcinoma after radical operation.Methods Seventy-four patients with cholangiocarcinoma radical operation admitted to our hospital from August 2017 to December 2018 were enrolled in the study,they were divided into control group (n=37) and study group (n=37) according to the random number table method.The control group was treated with intravenous analgesia pump analgesia.The study group underwent ultrasound-guided transabdominal transverse muscle plane nerve block combined with intravenous analgesia pump analgesia.The pain visual analog scale (VAS) scores before and after surgery,the incidence of postoperative analgesic drug adverse reactions,postoperative recovery and psychological status before and after surgery were compared.Results The VAS scores of the study group at 2,6,12,and 24 h after operation were lower than those of the control group,the differences were statistically significant (P<0.05).The total incidence of postoperative analgesic drug adverse reactions in the study group was lower than that in the control group,the difference was statistically significant (P<0.05).The time of getting out of bed,exhaust time and recovery time were shorter in the study group than those of the control group,the differences were statistically significant (P<0.05).The positive emotion scores of the study group at 24 h after operation were higher than those of the control group,and the negative emotion scores were lower than those of the control group,and the differences were statistically significant (P<0.05).Conclusion Ultrasound-guided transabdominal transverse muscle plane nerve block combined with intravenous analgesia pump can improve the pain level of patients after cholangiocarcinoma radical resection,reduce the occurrence of analgesic drug adverse reactions,and regulate the psychological state of patients.
