腹腔镜与开腹宫颈癌根治术联合腹主动脉旁淋巴结清扫治疗早期宫颈癌的效果对比

摘要目的比较腹腔镜与开腹宫颈癌根治术联合腹主动脉旁淋巴结清扫治疗早期宫颈癌的临床效果。方法选取2013年12月～2015年12月我院收治的90例早期宫颈癌患者作为研究对象，应用随机抽签法分为对照组和观察组，每组各45例。对照组应用开腹宫颈癌根治术联合腹主动脉旁淋巴结清扫进行治疗，观察组应用腹腔镜联合腹主动脉旁淋巴结清扫进行治疗，观察两组患者的手术情况、术后并发症及生存率情况，比较两组的治疗效果。结果观察组患者的排气时间、引流管拔除时间短于对照组，术中出血量少于对照组，差异有统计学意义（P＜0.05）；观察组患者术后并发症总发生率低于对照组，差异有统计学意义（P＜0.05）；两组患者术后1、2、3年的生存率比较，差异无统计学意义（P＞0.05）。结论腹腔镜联合腹主动脉旁淋巴结清扫治疗早期宫颈癌患者的效果显著，可以缩短排气时间、引流管拔除时间，减少术中出血量，且术后并发症情况较少，安全性较高，疗效可靠，具有临床应用价值。

Abstract：Objective To compare the clinical effects of laparoscopic and open radical resection of cervical cancer combined with para-aortic lymph node dissection in the treatment of early cervical cancer.Methods Ninety patients with early cervical cancer who were admitted to our hospital from December 2013 to December 2015 were selected as the subjects,and divided into the control group and an observation group by random sampling,with 45 cases in each group.The control group was treated with open radical laparoscopic radical resection combined with para-aortic lymph node dissection.The observation group was treated with laparoscopic and para-aortic lymph node dissection.The operation,postoperative complications and survival rate of the two groups were observed,and the treatment effect was compared.Results The exhaust time,drainage tube removal time of the observation group were shorter than those of the control group,and intraoperative bleeding volume of the observation group was less than that of the control group,and the differences were statistically significant(P＜0.05).The total incidence of complications in the observation group was lower than that in the control group (P＜0.05).There was no significant difference in 1,2 and 3 years survival rate between the two groups (P＞0.05).Conclusion Laparoscopy combined with para-aortic lymph node dissection for the treatment of patients with early cervical cancer is effective,can shorten exhaust time,drainage tube extraction time,and reduce intraoperative blood loss,and postoperative complications are less,and has higher safety.It has clinical application value.
