改良尿道板纵切卷管法在治疗小儿尿道下裂中的应用效果

摘要目的探讨改良尿道板纵切卷管法在治疗小儿尿道下裂的效果。方法回顾性分析2016年3月～2018年6月我院收治的47 例小儿尿道下裂患儿的手术资料，均采用改良尿道板纵切卷管法治疗（观察组），并按照1∶1 配比原则选取同期采用经典尿道板纵切卷管尿道成形术治疗的47 例患儿作为常规组，统计两组手术效果，并比较术后3～6 个月两组术后并发症发生情况，分析不同阴茎下曲分度的手术成功率。结果观察组手术总有效率为91.5%，常规组为89.4%，两组差异无统计学意义（P＞0.05）；观察组并发症总发生率为4.3%，常规组为12.8%，两组差异无统计学意义（P＞0.05）；观察组阴茎中重度下曲的首次成功率为88.2%，高于常规组的55.0%，差异有统计学意义（P<0.05），但两组轻度下曲患儿的手术成功率比较，差异无统计学意义（P＞0.05）。结论改良尿道板纵切卷管法能有效提高中度阴茎下曲的小儿尿道下裂手术首次成功率，且未增加不良反应，治疗效果较佳，值得临床推广。

Abstract：Objective To explore the effect of modified longitudinal incision and reeling of urethral plate in the treatment of hypospadias in children.Methods The operative data of 47 children with hypospadias admitted to our hospital from March 2016 to June 2018 were retrospectively analyzed.All the children were treated with improved longitudinal incision and tube-rolling method of urethral plate (observation group).According to the principle of 1∶1 ratio, 47 children with hypospadias who were treated with traditional longitudinal incision at the same time were selected as the routine group.The operative efficacy of the two groups was counted and the complications of the two groups were compared between 3 and 6 months after operation.In addition, the success rates of different degrees of subpenile curvature were analyzed.Results The total effective rate was 91.5% in the observation group and 89.4% in the routine group.There was no significant difference in the total effective rate between the two groups (P＞0.05).The total incidence of complications was 4.3% in the observation group and 12.8% in the routine group.There was no significant difference in the incidence between the two groups (P＞0.05).The first success rate of moderate and severe hypoplasia in the observation group was 88.2%, higher than 55.0% in the conventional group (P<0.05), but there was no significant difference between the two groups in the success rate of mild hypoplasia (P＞0.05).Conclusion The improved longitudinal incision of urethral plate and tube rolling method can effectively improve the success rate of hypospadias surgery in children with moderate hypospadias, without increasing adverse reactions, and has a better curative effect, which is worthy of clinical promotion.
