优质护理干预减少晚期癌痛患者阿片类药物不良反应发生率及提高生活质量的临床效果

摘要目的探讨护理干预在降低晚期癌痛患者阿片类药物不良反应发生率及提高生活质量中的作用。方法选择2017年7月～2018年7月我院收治的120 例晚期癌症患者，按随机数字表法分为观察组和对照组，每组60 例。对照组患者给予常规护理，观察组患者在晚期癌症常规护理的基础上加以优质护理干预。对比两组患者的不良反应发生率、生活质量评分及焦虑抑郁情况。结果观察组患者的不良反应发生率为28.33%，低于对照组的50.00%，差异有统计学意义（P＜0.05）。观察组的躯体、情绪、社会、认知和角色功能评分、整体生活质量评分高于对照组，差异有统计学意义（P＜0.05）。两组患者护理后的SDS 评分、SAS 评分均低于护理前，观察组护理后的SDS 评分、SAS 评分低于对照组，差异均有统计学意义（P＜0.05）。结论优质护理干预能降低阿片类药物带来的尿潴留、呼吸抑制等不良反应发生率，提高晚期癌痛患者的生活质量，改善患者焦虑抑郁，值得临床推广应用。

Abstract：Objective To explore the clinical effect of high-quality nursing intervention on reducing the incidence of adverse reaction of opioid and improving quality of life in patients with advanced cancer pain.Methods A total of 120 patients with advanced cancer who were admitted to our hospital from July 2017 to July 2018 were selected and divided into the observation group and the control group according to random number table method, 60 cases in each group.The patients in the control group were given routine nursing, and the observation group received high-quality nursing intervention on the basis of the advanced cancer routine nursing.The incidence of adverse reactions, quality of life scores and anxiety and depression were compared between the two groups.Results The incidence of adverse reactions in the observation group was 28.33%, which was lower than 50.00% in the control group, the difference was statistically significant (P＜0.05).The scores of physical, emotional, social, cognitive and role functions and overall quality of life in the observation group were higher than those in the control group, and the difference was statistically significant (P＜0.05).The SDS score and SAS score of the two groups after nursing were lower than those before nursing, while the SDS score and SAS score of the observation group after nursing were lower than those of the control group, the difference was statistically significant (P＜0.05).Conclusion High-quality nursing intervention can reduce the incidence of adverse reactions such as urinary retention and respiratory inhibition caused by opioid drugs, improve the quality of life of patients with advanced cancer pain and improve the anxiety and depression of the patients.It is worthy of clinical promotion and application.
