低蛋白血症对慢性心力衰竭预后的影响分析

摘要目的研究低蛋白血症对慢性心力衰竭患者预后的影响。方法从2013年3月～2015年3月于航空总医院心内科住院的患者中，选取确诊为慢性心力衰竭，心功能Ⅱ～Ⅳ级的患者94例，将上述患者按照血清清蛋白水平分为三组：正常组48例、轻度低蛋白血症组28例、中重度低蛋白血症组18例。采用前瞻性研究的方法，在接受常规治疗心力衰竭情况下，追踪观察2年，收集有关资料，分析血清清蛋白水平对慢性心力衰竭患者再住院率、病死率以及心功能指标的影响。结果合并低蛋白血症的慢性心力衰竭患者的再住院率、病死率高于正常组，差异有统计学意义（P＜0.05）。中重度低蛋白血症的慢性心力衰竭患者的再住院率、病死率高于轻度低蛋白血症组，差异有统计学意义（P＜0.05）。正常组患者2年后的N端前脑钠肽（NT-proBNP）、左室射血分数（LVEF）、6 min步行距离、左室舒张末径（LVEDV）较治疗前明显改善，差异有统计学意义（P＜0.05）。低蛋白血症组患者2年后的上述指标较治疗前恶化，差异有统计学意义（P＜0.05）。清蛋白水平越低，心功能指标越差。结论慢性心力衰竭合并低蛋白血症的患者再住院率及病死率较高，且清蛋白水平越低，其病情越严重，预后越差。

Abstract：Objective To study of the effect of hypoproteinemia on the prognosis of patients with chronic heart failure.Methods 94 patients who diagnosed with chronic heart failure withⅡ-Ⅳgrade of cardiac function were selected from March 2013 to March 2015 of the Department of Cardiovascular Medicine in General Hospital of China Aviation,and patients were divided into the 3 groups according to the level of serum albumin:48 cases in normal group,28 cases in mild hypoproteinemia and 18 cases in moderate and severe hypoproteinemia.A prospective study was carried out for 2 years after routine treatment of heart failure.The data were collected.The effect of serum albumin level on rehospitalization rate,case fatality rate and related indexes of cardiac function in patients with chronic heart failure were analyzed.Results The rate of rehospitalization and case fatality rate in patients with chronic heart failure with hypoproteinemia was higher than that in the normal group,and the difference was statistically significant(P＜0.05).The rate of rehospitalization and mortality in patients with moderate and severe hypoproteinemia with chronic heart failure was higher than that in the mild hypoproteinemia group,and the difference was statistically significant(P＜0.05).After 2 years,the N terminal pro brain natriuretic peptide(NT-proBNP),left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDV)and 6 min walking distance in normal group were significantly improved compared with before treatment,and the difference was statistically significant(P＜0.05).After 2 years,the above indexes of the hypoproteinemia group were worse than those before treatment,and the difference was statistically significant(P＜0.05).The lower the albumin level,the worse the cardiac function index.Conclusion Patients with chronic heart failure and hypoproteinemia have a higher rate of rehospitalization and case fatality rate,and the lower the albumin level,the more serious the patient′s condition is and the worse the prognosis.
