腹腔镜手术联合促性腺激素释放激素激动剂治疗卵巢巧克力囊肿合并不孕的临床效果观察

摘要目的探讨腹腔镜手术联合促性腺激素释放激素激动剂（GnRHa）治疗卵巢巧克力囊肿合并不孕的临床效果。方法对2016年5月～2017年1月在我院就诊的卵巢巧克力囊肿合并不孕82例患者进行前瞻性研究，依据随机数字表法随机分为对照组（n=41，予以腹腔镜手术治疗）与观察组（n=41，予以腹腔镜手术联合GnRHa治疗）。观察两组的临床疗效、妊娠情况及治疗前后的激素水平、临床症状积分变化与不良反应情况。结果观察组的总有效率及Ⅲ～Ⅳ期患者妊娠率高于对照组，差异有统计学意义（P＜0.05）；治疗前，两组的激素水平、症状积分比较，差异无统计学意义（P＞0.05），治疗后，两组的临床症状积分均低于治疗前（P＜0.05），且观察组治疗后的症状积分均低于对照组，差异有统计学意义（P＜0.05）；治疗后，两组的LH、FSH水平均高于治疗前，E2、P水平均低于治疗前，差异均有统计学意义（P＜0.05），且治疗后，观察组的LH、FSH水平均高于对照组，E2、P水平均低于对照组，差异有统计学意义（P＜0.05）。观察组患者予第2次用药后出现低雌激素水平症状的不良反应，但多可耐受，症状严重者予以对症处理后缓解。结论卵巢巧克力囊肿合并不孕患者采用腹腔镜手术联合GnRHa治疗能提高疗效，降低术后复发率，提高妊娠率，改善卵巢功能，具有推广价值。

Abstract：ObjectiveTo investigate the effect of laparoscopic surgery combined with Gonadotropin-Releasing Hormone Agonist(GnRHa)in the treatment of ovarian chocolate cystwith infertility.MethodsA prospective study of 82 patients with ovarian chocolate cysts and infertility who were treated in our hospital from May 2016 to January 2017 wasmade.Allpatientswere randomly divided into the controlgroup(n=41,treatedwith laparoscopic surgery)and theobservation group(n=41,treated with laparoscopic surgery combined with GnRHa).The clinical effect,reproductive outcome,pre-and posttreatment hormone levels,changes in clinical symptom scores and adverse reactions were observed in the two groups.ResultsThe total effective rate and pregnancy rate in phaseⅢ-Ⅳ of the observation group were higher than those of the controlgroup,with statistically significantdifferences(P＜0.05).Therewas no statistically significant difference between the two groups in pre-treatment hormone levels and symptom scores(P＞0.05),but the post-treatment clinical symptoms scoresof the two groupswere lower than those before treatmentwith statistically significantdifference(P＜0.05).The posttreatment symptom score of the observation group was lower than that of the control group,with statistically significant difference(P＜0.05).The post-treatment luteinizing hormone(LH)and follicle stimulating hormone(FSH)of two groups were higher than those before the treatment,with statistically significant difference(P＜0.05),and the level of post-treatment estradiol(E2)and progesterone(P)of two groupswere lower than before the treatment,with statistically significant difference　(P＜0.05).The post-treatment LH and FSH of the observation group were higher than those of control group,and post-treatment E2and P levels of the observation group were lower than those of control group,both with statistically significant differences(P＜0.05).The adverse reactions of low estrogen level symptoms appeared in patients of the observation group after second medication but were well tolerated,and those with severe symptoms were relieved after treatment.ConclusionApplication of combination of laparoscopic surgery and GnRHa in the treatment of ovarian chocolate cystwith infertility can improve the curative effect,the pregnancy rate and ovarian functions,and reduce the postoperative recurrence rate,which isworth promoting.
