Advances in the application of elevated cardiac troponin in patients with chronic kidney disease

Abstract：Cardiac troponin(cTn)can be increased at the time of myocardial necrosis,which,as is universally known,it is a marker for diagnosis of myocardial injury,especially in the diagnosis of acute coronary syndrome(ACS)that have been recognized by at home and abroad.However,the increase of cTn is also seen in non-coronary heart diseases(myocarditis,arrhythmia,heart failure)and non-heart diseases(chronic kidney disease,sepsis,stroke etc.).In recent years,it has no longer been something unusual that significantly higher serum cTn is detected in patients with chronic kidney disease (CKD)than that of non-CKD patients.As for the reason,on the one hand,the elevated levels of cTn is subject to kidney diseases,such as decreased renal clearance rate;on the other hand,CKD complicated with cardiovascular diseases(CVD) eventually leads to the rise of cTn levels.In this paper,the causes and mechanisms of elevated cTn in non-dialysis CKD, the relationship between cTn and kidney diseases as well as its role in prediction of CVD with ACS are mainly discussed.
