心理干预对晚期非小细胞肺癌化疗患者生活质量的效果研究

摘要目的探讨心理干预对晚期非小细胞肺癌化疗患者生活质量的效果。方法选择2014年1月~2015年12月我院肿瘤内科收治的80例晚期非小细胞肺癌患者纳入本研究,根据入院单双号分为对照组和观察组,各40例。在化疗期间给予对照组常规护理,在此基础上给予观察组心理干预。比较两组化疗前、化疗2个周期患者生活质量、抑郁和焦虑情况、遵医行为和近期疗效。结果化疗前,两组患者EORTC QLQ-C30各项指标评分、SAS、SDS评分结果比较,差异均无统计学意义（P>0.05）;化疗后两组患者的EORTC QLQ-C30各项指标评分均有所提高,SAS、SDS评分所有下降,与化疗前比较,差异有统计学意义（P<0.05）;观察组EORTC QLQ-C30各项指标评分均高于对照组,SAS、SDS评分低于对照组,差异有统计学意义（P<0.05）。观察组患者遵医行为高于对照组,疾病控制率高于对照组,差异有统计学意义（P<0.05）。两组患者治疗总有效率比较,差异无统计学意义（P>0.05）。结论心理干预能够提高晚期非小细胞肺癌患者治疗依从性,缓解焦虑和抑郁情绪,提高生活质量和疾病控制率。

Abstract：Objective To explore the efficacy of psychological intervention on the quality of life of patients with advanced non-small cell lung cancer.Methods 80 patients with advanced NSCLC enrolled in our department of oncology from January 2014 to December 2015 were selected and divided into the control group and the observation group according to the single and double numbers of admission,with 40 cases in each group. The control group was given routine nursing care during the chemotherapy period,and the observation group was given psychological intervention on the basis of the control group.The quality of life,depression and anxiety of the patients before chemotherapy and 2 cycles after chemotherapy,the compliance behavior and short-term efficacies were compared between two groups.Results Before chemotherapy,there were no significant differences among the scores of indexes in EORTC QLQ-C30 and scores of SAS and SDS in two groups (P>0.05);after chemotherapy,the scores of indexes in EORTC QLQ-C30 in two groups were all increased,the scores of SAS and SDS were all decreased,and compared with those before chemotherapy,the differences were statistically significant (P<0.05).And the scores of indexes in EORTC QLQ-C30 in the observation group was higher than those in the control group,the scores of SAS and SDS was lower than those in the control group,and the differences were statistically significant (P<0.05).The compliance behavior in the observation group was higher than that in the control group,the disease control rate was higher than that in the control group,and the differences were statistically significant (P<0.05).There was no significant differences in the total effective rate between two groups (P>0.05).Conclusion Psychological intervention can improve the treatment compliance of patients with advanced non-small cell lung cancer,relieve anxiety and depression,improve the quality of life and disease control rate.
