Clinical aPPlication of color DoPPler ultrasound in early diagnosis of newborn hyPoxic-ischemic encePhaloPathy

Abstract:Objective To exp1ore c1inica1 va1ue of co1or Dopp1er u1trasound in ear1y diagnosis of newborn hypoxic-ischemic encepha1opathy. Methods 68 newborns born in department of obstetrics and gyneco1ogy of Ci County Peop1e's Hospita1 and diagnosed as HIE from January 2012 to September 2014 were se1ected as study object.Patients were divided into s1ight group,median group and serve group according to c1inica1 grading,and 40 hea1thy newborns were se1ected as hea1th group.Co1or dopp1er u1trasound was used to dynamic monitor the change of brain parenchyma echo and hemodynamic parameters 1iked Vs,Vd and RI of cerebra1 midd1e artery of 108 newborns after 24 h,48 h and 72 h birth.The difference of that index among four groups was compared,and its diagnostic resu1t was compared with CT,MRI. Results The index of Vs and Vd in hea1thy group at 24 h and 48 h birth was higher than that of i11ed chi1dren the three groups respective1y;among i11ed chi1dren in three groups,the index of Vs and Vd in s1ight group at 24 h and 48 h birth was higher than that of median group and serve group respective1y,whi1e median group was higher than that of serve group,and the difference was statistica1 significance between the groups (P<0.05).The index of RI in hea1th group at 24 h and 48 h birth was 1ower than that of i11ed chi1dren in three groups,the index of RI in s1ight group at 24 h and 48 h birth was 1ower than that of median group and serve group,whi1e median group was 1ower than that of serve group,and the difference was statistica1 significance between the groups (P<0.05).The index of Vs,Vd and RI had no statistica1 significance at 72 h birth among four groups (P>0.05).Among 68 i11ed patients,8 s1ight i11ed chi1dren did not see any obvious abnormity in the u1trasound resu1ts,and the tota1 diagnosis rate was 88.24%. Conclusion Dopp1er u1trasound resu1ts of newborns MAC hemodynamics can be an important basis for ear1y diagnosis of HIE.
