Abstract：Objective To explore the application effect of lateral decubitus position delivery in primipara with persistent occipitotransverse and occipitoposterior.Methods 100 single-fetus full-term primiparas with persistent occipitotransverse and occipitoposterior in our hospital from May 2013 to October 2015 were selected and randomly divided into the observation group and the control group，50 cases in each group.The observation group was managed by lateral decubitus position delivery in the second stage of labor，the control group was managed by conventional supine lithotomy position.The time of the second stage of labor，mode of delivery，the rate of lateral episiotomy，the rate of perineal laceration，correction of fetal position，the rate of neonatal asphyxia and satisfaction of patients towards delivery outcome was compared respectively.Results The time of the second stage of labor in the observation group was（46.52±11.56）hours，which was shorter than（62.33±14.56）hours in the control group，with significant difference（P＜0.01）.The proportion of correction from persistent occipitotransverse and occipitoposterior to normal fetal position in the observation group was 86.00%，which was higher than 64.00%in the control group，with significant difference （P＜0.05）.The rate of vaginal delivery in the observation group was 88.00%，which was higher than 62.00%in the control group，with significant difference （P＜0.01）.The rate of lateral episiotomy in the observation group was lower than that in the control group，with significant difference（P＜0.01）.There was no significant difference in the rate of neonatal asphyxia and the rate of perineal laceration between the two groups （P＞0.05）.The overall satisfaction degree of patients towards delivery outcome was 98.00%in the observation group，which was higher than 78.00%in the control group，with significant difference （P＜0.01）.Conclusion Lateral decubitus position can effectively correct the fetal position in primiparas with persistent occipitotransverse and occipitoposterior，shorten the second stage of labor，improve the rate of vaginal delivery and reduce the rate of lateral episiotomy，improve the satisfaction of patients towards delivery mode with significant clinical effect.
