基于加速康复外科理念的护理干预在胃癌根治术患者中的应用

摘要目的探讨基于加速康复外科（FTS）理念的护理干预在胃癌根治术患者的应用效果。方法选择2018年5月～2020年5月广东省茂名市人民医院的106 例胃癌根治术患者，按随机数字表法分为对照组（53 例）和观察组（53例）。对照组采取常规护理，观察组实施FTS 护理干预。比较两组的康复情况，干预前和干预7 d 后营养状态[血清白蛋白（ALB）]、炎症因子[C-反应蛋白（CRP）、白介素-6（IL-6）]水平，观察并比较两组的并发症发生率。结果观察组的肛门排气时间、首次下床时间、住院时间短于对照组，差异均有统计学意义（P＜0.05）。干预前两组的营养状况比较，差异无统计学意义（P＞0.05）。观察组干预后的ALB 高于对照组，CRP、IL-6 低于对照组，差异均有统计学意义（P＜0.05）。观察组的并发症总发生率低于对照组，差异有统计学意义（P＜0.05）。结论 FTS 护理能改善胃癌根治术患者营养状况，减少并发症发生，而促进患者康复。

Abstract：Objective To explore the application effect of nursing intervention based on the concept of accelerated rehabilitation surgery(FTS)in patients with gastric cancer radical operation.Methods A total of 106 patients with gastric cancer undergoing radical gastrectomy in Maoming People′s Hospital in Guangdong Province from May 2018 to May 2020 were selected and divided into the control group(53 cases)and the observation group(53 cases)according to random number table method.The control group was given routine nursing,while the observation group was given FTS nursing intervention.The levels of nutritional status(serum albumin [ALB],inflammatory factors C-reactive protein[CRP],interleukin-6 [IL-6])before and after the intervention were compared between the two groups,and the incidence of complications was observed and compared between the two groups.Results The anal exhaust time,first time out of bed time and hospitalization time of the observation group were shorter than those of the control group,the differences were statistically significant(P＜0.05).There was no significant difference in nutritional status between the two groups before intervention(P＞0.05).After intervention,ALB in the observation group was higher than that in the control group,CRP and IL-6 in the observation group were lower than those in the control group,the differences were statistically significant(P＜0.05).The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P＜0.05).Conclusion FTS nursing can improve the nutritional status of patients with gastric cancer radical operation,reduce the incidence of complications,so as to promote the rehabilitation of patients.
