Diagnostic value of alveolar-arterial blood oxygen pressure difference in pneumoconiosis

Abstract:Objective To explore the diagnostic value of alveolar-arterial blood oxygen pressure (A-aDO2) difference in pneumoconiosis. Methods A total of 40 pneumoconiosis patients in Shaoguan occupational disease prevention and control hospital from January 2020 to December 2020 were selected as the observation group, and 40 patients with the same dust contact conditions admitted in Shaoguan Occupational Disease Prevention Hospital from January to December 2020 were selected as control group. The partial pressure of carbon dioxide (PaCO2), arterial oxygen partial pressure(PaO2), A-aPO2, forced expiratory volume in one second (FEV1), and forced vital capacity (FVC) values of the two groups were detected, the ROC curve was drawn to analyze the diagnostic efficacy of PaCO2, PaO2 and A-aDO2. Results PaO2 in the observation group was lower than that in the control group, with statistical significant difference (P<0.05). PaCO2 and A-aDO2 in the observation group were higher than those in the control group, with statistical significant differences (P<0.05). FEV1 and FVC in the observation group were lower than those in the control group (P<0.05).The PaO2, PaCO2, A-aDO2 of pneumoconiosis patients′ AUC and sensitivity were 0.788 and 86%, 0.863 and 87%,0.873 and 98%. Conclusion A-aDO2 has good diagnostic value in pneumoconiosis patients. The AUC and sensitivity of A-aDO2 are significantly higher than those of PaCO2 and PaO2. Early monitoring of A-aDO2 is helpful to early diagnose pneumoconiosis, evaluate the pulmonary function of pneumoconiosis patients, judge the progress and prognosis of the disease, so as to treat pneumoconiosis in time.
