品管圈活动在剖宫产初产妇母乳喂养自我效能提升中的应用

摘要目的探讨品管圈（QCC）活动在剖宫产初产妇母乳喂养自我效能提升中的应用效果。方法选取2019年1月～2020年6月于抚州市妇幼保健院生产的86 例初产妇作为研究对象，按随机数字表法将其分为对照组与观察组，每组各43 例。对照组产妇采取常规母乳喂养健康教育，观察组产妇在对照组的基础上开展QCC 活动。比较两组产妇干预前和干预7 d 的母乳喂养自我效能感量表（BSES）评分、首次母乳喂养时间、平均每日喂养次数、出院30 d后的母乳喂养率和并发症发生情况。结果干预前，两组产妇的BSES 评分比较，差异无统计学意义（P>0.05）。干预7 d 后，观察组产妇的BSES 个人内心感受[（65.19±6.14）分]、喂养技能[（60.20±6.24）分]评分高于对照组[（56.87±6.02）、（53.67±5.28）分]，差异有统计学意义（P<0.05）。观察组产妇的首次母乳喂养时间[（2.57±0.34）d]短于对照组[（4.01±1.20）d]，平均每日喂养次数[（9.67±2.38）次]多于对照组[（8.09±2.35）次]，差异有统计学意义（P<0.05）。观察组产妇的母乳喂养率（95.35%）高于对照组（81.40%），并发症总发生率（4.65%）低于对照组（18.60%），差异有统计学意义（P<0.05）。结论 QCC 活动能够有效增强剖宫产初产妇的母乳喂养信心，提高母乳喂养率，降低产后并发症发生率，对于产妇产后恢复和新生儿健康成长具有积极意义。

Abstract：Objective To explore the application value of quality control circle(QCC)activities in improving self-efficacy of breast-feeding in primipara after cesarean section.Methods A total of 86 primiparas admitted to Fuzhou Maternal and Child Health Care Hospital from January 2019 to June 2020 were selected as the research objects and divided into the control group and the observation group according to random number table method,with 43 cases in each group.The control group received routine health education of breastfeeding,and the observation group received QCC activities on the basis of the control group.Breastfeeding self-efficacy scale(BSES)score,time of first breastfeeding,average daily feeding times,breastfeeding rate and complications 30 days after discharge were compared between the two groups before intervention and 7 d after intervention.Results Before intervention,there was no significant difference in BSES scores between the two groups(P>0.05).After 7 days of intervention,BSES personal inner feeling([65.19±6.14]points)and feeding skills([60.20±6.24]points)in the observation group were higher than those in the control group([56.87±6.02]and[53.67±5.28]points),and the differences were statistically significant(P<0.05).The time of the first breastfeeding in the observation group([2.57±0.34]d)was shorter than that in the control group([4.01±1.20]d),and the average daily feeding times([9.67±2.38]times)was more than that in the control group([8.09±2.35]times),the differences were statistically significant(P<0.05).The rate of breastfeeding in the observation group(95.35%)was higher than that in the control group(81.40%),and the total incidence of complications in the observation group(4.65%)was lower than that in the control group(18.60%),the differences were statistically significant(P<0.05).Conclusion The activities of QCC can effectively enhance the confidence of breastfeeding,improve the rate of breastfeeding,reduce the incidence of postpartum complications,and have positive significance for the recovery of postpartum and the healthy growth of the newborn.
