左甲状腺素钠片治疗妊娠期合并甲状腺功能减退的临床效果及对患者甲状腺激素水平变化的观察

摘要目的分析左甲状腺素钠片治疗妊娠期合并甲状腺功能减退（妊娠期甲减）的临床效果及其对患者甲状腺激素水平变化的影响。方法选取2018年7月—2019年4月于锦州市妇婴医院就诊的46 例妊娠期甲减患者作为研究对象，以抛硬币的方式将其分为对照组（23 例）和治疗组（23 例）。对照组采用桂附地黄丸治疗，治疗组采用左甲状腺素钠片治疗，比较两组的治疗效果及血清促甲状腺激素（TSH）、游离三碘甲状腺原氨酸（FT3）、血清游离甲状腺素（FT4）水平。结果治疗组的临床治疗总有效率（95.65%）高于对照组（69.57%），差异有统计学意义（P<0.05)。治疗前，两组TSH、FT3、FT4 水平比较，差异无统计学意义（P>0.05）；治疗后两组的TSH 水平低于治疗前，FT3、FT4 指标高于治疗前，且治疗组的TSH 指标水平低于对照组，FT3、FT4 指标水平高于对照组，差异有统计学意义（P<0.05）。结论应用左甲状腺素钠片治疗妊娠期甲减患者，可改善甲状腺激素水平，治疗效果显著。

Abstract：Objective To analyze the clinical effect of Levothyroxine Sodium Tablets in the treatment of hypothyroidism during pregnancy and its influence on the change of thyroid hormone level. Methods A total of 46 patients with hypothyroidism during pregnancy who were treated in Jinzhou Maternal and Child Hospital from July 2018 to April 2019 were selected as the research subjects, and they were divided into control group (23 cases) and treatment group (23 cases) by coin toss. The control group was treated with Guifu Dihuang Pills and the treatment group was treated with Levothyroxine Sodium Tablets.The therapeutic effect and the levels of thyroid stimulating hormone(TSH),free triiodothyronine (FT3) and free thyroxine (FT4) were compared between two groups. Results The total effective rate of the treatment group (95.65%) was higher than that of the control group (69.57%), and the difference was statistically significant(P<0.05). Before treatment, there was no significant difference in TSH, FT3 and FT4 levels between the two groups (P>0.05). After treatment, TSH level of the two groups was lower than before treatment, FT3 and FT4 indexes were higher than before treatment, and TSH index level of the treatment group was lower than the control group, FT3 and FT4 indexes were higher than the control group, the differences were statistically significant (P<0.05). Conclusion Levothyroxine Sodium Tablets can improve the level of thyroid hormone in patients with hypothyroidism during pregnancy, and the therapeutic effect is remarkable.
