子宫肌瘤剔除术后残留复发的影响因素分析

摘要目的分析子宫肌瘤剔除术后残留复发的影响因素，为子宫肌瘤剔除术后残留复发的防治提供依据。方法回顾性分析2017年2月—2020年3月南通市妇幼保健院及启东市人民医院68 例行子宫肌瘤剔除术患者的临床资料，统计所有患者术后6 个月残留复发情况，收集患者临床资料，包括年龄、手术方式、肌瘤数目、肌瘤直径、肌瘤位置、肌瘤类型、血清泌乳素（PRL）、基质金属蛋白酶-9（MMP-9）水平，分析子宫肌瘤剔除术后残留复发的影响因素。结果 68 例患者中，术后残留复发7 例，发生率为10.29%。残留复发组与无残留复发组的年龄、肌瘤数目、肌瘤位置、MMP-9 水平比较，差异有统计学意义（P＜0.05）；经多因素Logistic 回归分析发现，年龄＜40 岁、肌瘤数目≥4 个、肌壁间肌瘤、MMP-9 水平≥200 ng/L 均为子宫肌瘤剔除术后残留复发的影响因素（P＜0.05）。结论子宫肌瘤患者行剔除术后具有一定的残留复发率，应针对术后残留复发的影响因素进行干预，改善患者预后。

Abstract：Objective The influencing factors of residual recurrence after hysteromyomectomy were analyzed to provide the basis for the prevention and treatment of residual recurrence after hysteromyomectomy. Methods The clinical data of 68 patients who underwent hysteromyomectomy in the Nantong Maternal and Child Health Care Hospital and Qidong Maternal and Child Health Care Hospital from February 2017 to March 2020 were retrospectively analyzed. Residual recurrence of all patients within 6 months after surgery was counted. Clinical data of patients were collected, including age, surgical method, number of fibroids, fibroid diameter, fibroid location, fibroid type, serum prolactin (PRL) and matrix metalloproteinase-9 (MMP-9) levels, analyzed the influencing factors of residual recurrence after hysteromyomectomy. Results Among the 68 patients, postoperative residual recurrence occurred in 7 cases (10.29%). Age, number of fibroids, location of fibroids and MMP-9 level were compared between the residual or recurrent group and the nonresidual or recurrent group, the differences were statistically significant (P<0.05). Multivariate Logistic regression analysis found that Age<40 years old, number of fibroids≥4, intramural fibroids, MMP-9 level≥200 ng/L were all the influencing factors for residual recurrence after hysteromyomectomy (P<0.05). Conclusion Patients with uterine fibroids have a certain residual recurrence rate after removal, and intervention should be carried out according to the influencing factors of postoperative residual recurrence to improve the prognosis of patients.
