经阴道彩超联合经腹部彩超诊断妇产科急腹症的应用效果

摘要目的研究分析经阴道彩超联合经腹部彩超诊断妇产科急腹症的应用效果。方法选取2018年9月～2019年9月于辽宁省铁岭市中心医院妇产科就诊的122例急腹症患者作为研究对象，采用随机数字表法分为观察组和常规组，各61例。常规组患者采用经腹部彩超进行诊断，观察组则采用经阴道彩超联合经腹部彩超进行诊断。以病理检查为诊断金标准，比较两组患者的诊断符合率。结果观察组患者的诊断总体符合率为96.72%，高于常规组的78.69%，差异有统计学意义（P＜0.05）。观察组患者中卵巢黄体破裂、胎盘早剥、急性盆腔炎、卵巢肿瘤蒂扭转4种妇产科急腹症的诊断符合率均高于常规组，差异均有统计学意义（P＜0.05）。两组患者在异位妊娠诊断中的符合率比较，差异无统计学意义（P＞0.05）。结论运用经腹部彩超联合经阴道彩超进行妇产科急腹症的诊断，能更好地明确病因，提高诊断符合率，具有较好的临床应用价值。

Abstract：Objective To study and analyze the application effect of transvaginal and transabdominal color Doppler abdomen in obstetrics and gynecology.Methods From September 2018 to September 2019,a total of 122 patients with acute abdomen from Liaoning Tieling Central Hospital were selected as subjects.They were divided into observation group and routine group by the random digital table method,61 cases in each group.Patients in the routine group were diagnosed by transabdominal color Doppler ultrasound,while the patients in the observation group were diagnosed by transvaginal color Doppler ultrasound combined with transabdominal color Doppler ultrasound.According to the diagnostic criteria of pathological examination,the diagnostic coincidence rate of the two groups was compared with pathological examination.Results The overall diagnostic coincidence rate of the patients in the observation group was 96.72%,it was higher than that in the routine group which was 78.69%,and the difference was statistically significant(P＜0.05).In the observation group,the diagnostic coincidence rates of the four types of obstetrics and gynecology acute abdomen,including ruptured corpus luteum,abruption of placenta,acute pelvic inflammatory disease,and torsion of ovarian tumor pedicle,were all higher than those in the routine group,and the differences were statistically significant(P＜0.05).Compared with the two groups in the diagnosis of ectopic pregnancy,there was no significant difference(P＞0.05).Conclusion Transabdominal color Doppler ultrasound combined with transvaginal color Doppler ultrasound for diagnosis of acute abdomen in obstetrics and gynecology can better identify the etiology,improve the diagnostic coincidence rate,has a better clinical application value.
