儿童早期预警评分联合SBAR交班在儿科病房的临床应用

摘要目的探讨儿童早期预警评分联合SBAR交班在儿科病房住院患儿中的应用效果。方法选取2016年6月～2017年5月在本院儿科病房住院的符合标准的患儿120例。按照就诊先后顺序，将其随机分为研究组（常规诊治+儿童预警评分+SBAR交班，n＝60）与对照组（常规诊治，n＝60）。比较两组患儿的住院时间、交班问题发生情况和家属的总满意度。在两组患儿干预期间分别随机抽取50名医生，调查其工作满意度。结果研究组患儿家属满意度为98.3%，医生满意度为96.0%，分别高于对照组的86.7%、80.0%（P＜0.05）；研究组患儿住院时间较对照组短，差异有统计学意义（P＜0.05）；研究组交班问题发生率较对照组低，差异有统计学意义（P＜0.05）；研究组非计划入住PICU率、自动出院/死亡率均低于对照组，差异有统计学意义（P＜0.05）。结论在儿科病房管理中，实施儿童预警评分联合SBAR交班模式的效果理想，能提升家属和医生满意度，缩短患儿住院时间，减少交班问题的发生，控制非计划入住PICU率、自动出院/死亡率，值得进行深入研究和推广。

Abstract：ObjectiveTo explore the application effect of early warning score for children combined with SBAR shift in hospitalized children of pediatric ward.Methods120 pediatric patients in line with standards and hospitalized in pediatric ward of our hospital from June 2016 to May 2017 were selected.According to the order of admission to the hospital,they were divided into the study group (n＝60,given regular diagnosis treatment+early warning score for children+SBAR shift exchange)and control group(only regular treatment)in random.The hospital stay,the occurrence of shift exchange problems,and the family satisfaction degree were compared between the two groups.Fifty doctors were randomly investigated during the intervention of the two groups for their job satisfaction degree in pediatric department.ResultsIn the study group,the family satisfaction degree and doctor′s satisfaction degree was 98.3%,96.0%respectively,which was higher than that of the control group (86.7%,80.0%respectively,which were displayed statistical differences(P＜0.05).In the study group,the hospital stay was shorter in comparison with that in the control group with a statistical difference (P＜0.05).The incidence of shift exchange problems in the study group was lower than that in the control group with a statistical difference(P＜0.05).In the study group,the non-planned occupancy rate of PICU,the rate of discharging without medical advice,and mortality were all lower than those in the control group,which were displayed statistical differences (P＜0.05).ConclusionIn the management of pediatric wards,application of early warning score for children combined with SBAR shift mode can improve family and doctor satisfaction degree,shorten the hospital stay,reduce the occurrence of shift problems,and control non-planned PICU rate,the rate of discharging without medical advice,and mortality,which is worthy of further study and promotion.
