Application effect of continuous health management model via mobile phone WeChat in community type 2 diabetes mellitus

Abstract:Objective To explore the application effect of continuous health management model via WeChat public platform and WeChat group on the self-management ability and quality of life of type 2 diabetic patients in community.Methods According to the inclusion criteria, altogether 134 patients with type 2 diabetes mellitus admitted to Hudong Community Health Service Center in Pudong New District of Shanghai City from August 1, 2015 to July 31, 2017 were selected as the study subjects.The patients were randomly divided into control group and intervention group by random number table method, 67 cases in each group.All the patients were discharged after comprehensive treatment.The control group was given routine follow-up nursing after discharge.The intervention group adopted continuous health management based on WeChat public platform and WeChat group on the basis of the control group.The scores of selfmanagement ability, coping style and quality of life were compared between the two groups.Results The level of fasting blood glucose after mtervention of the intervention group was lower than that before the intervention, with statistically significant difference (P<0.05); and there was statistically significant difference between the intervention group and the control group in fasting blood glucose (P<0.05).After 6-month intervention, the level of postprandial 2 -hour blood glucose of intervention group was significantly lower than that of control group, with statistically significant difference (P<0.05).In terms of self-management ability,after 6-month intervention, there was statistically significant difference between the intervention group and the control group in self-management levels, such as medication compliance, blood glucose monitoring and reasonable diet (P<0.05).In terms of coping style, after 6-month intervention, the face score of intervention group was higher than that of control group, and the scores of avoidance and yield were lower than those of control group (P<0.05).In terms of quality of life score, after 6-month intervention, the quality of life score of intervention group was higher than that of control group (P<0.05).Conclusion Continuous health management based on WeChat public platform and WeChat group can effectively control blood glucose level, optimize coping style and improve quality of life in community patients with type 2 diabetes.
