Correlation study of insulin resistance and carotid intima-media thickness in diabetes mellitus/non-diabetes mellitus patients with peritoneal dialysis

摘要目的探讨糖尿病与非糖尿病腹膜透析患者胰岛素抵抗与颈动脉内中膜厚度的相关性。方法选取2016年6月～2017年6月在我院腹膜透析中心行腹膜透析的56例糖尿病与非糖尿病终末期肾病患者，其中糖尿病腹膜透析患者28例，非糖尿病腹膜透析患者28例。根据是否存在胰岛素抵抗，将入选患者分为胰岛素抵抗组和非胰岛素抵抗组，比较两组患者胰岛素抵抗与颈动脉内中膜厚度的相关性。结果糖尿病腹膜透析患者胰岛素抵抗与2 h PBG、CIMT、LDLC、TG、hs-CRP均呈正相关（P＜0.05），非糖尿病腹膜透析患者胰岛素抵抗与2 h PBG、CIMT、HbA1c、LDLC、TG、hs-CRP均呈正相关 （P＜0.05）。结论 糖尿病患者与非糖尿病腹膜透析患者胰岛素抵抗均与2 h PBG、CIMT、LDLC、TG、hs-CRP呈正相关，胰岛素抵抗会促进颈动脉内中膜厚度增厚。
Abstract：ObjectiveTo explore the correlation between insulin resistance and carotid intima-media thickness in diabetes mellitus(DM)/non-DM patients with peritoneal dialysis.MethodsFrom June 2016 to June 2017,56 end-stage renal disease(CHD)patients with DM/non-DM who were performed with peritoneal dialysis in our hospital were selected. There were 28 cases with DM and the rest were not occurred with DM.According to insulin resistance or not,they were divided into insulin resistance group and non-insulin resistance group.The correlation of insulin resistance and carotid intima-media thickness in the two groups was compared.ResultsFor DM patients,insulin resistance had positive correlations with 2 h PBG,CIMT,LDLC,TG and hs-CRP(P＜0.05).In the non-DM group,insulin resistance was positively correlated with 2 h PBG,CIMT,HbA1c,LDLC,TG and hs-CRP(P＜0.05).ConclusionInsulin resistance in DM/non-DM patients is both positively correlated with 2 h PBG,CIMT,LDLC,TG and hs-CRP,which indicates insulin resistance can promote the thickening of carotid intima-media thickness.
