超声检查在急性阑尾炎诊断中的应用价值

摘要目的探讨超声检查在急性阑尾炎诊断中的应用价值。方法选取2016年1月～2019年3月我院收治的60例疑似急性阑尾炎患者作为研究对象，患者均给予超声、电子计算机断层扫描（CT）检查，以手术病理诊断为金标准，比较两种检查方式的阳性预测值、阴性预测值、准确度、灵敏度、特异度。结果手术病理诊断急性阑尾炎阳性55 例，阴性5 例；超声检查的准确度为85.00%（51/60），高于CT 检查的70.00%（42/60），差异有统计学意义（P＜0.05）；超声检查的灵敏度[85.45%（47/55）]、特异度[80.00%（4/5）]、阳性预测值[97.92%（47/48）]及阴性预测值[33.33%（4/12）]与CT 检查[72.73%（40/55）、40.00%（2/5）、93.02%（40/43）、11.76%（2/17）]比较，差异无统计学意义（P＞0.05）。结论超声检查在急性阑尾炎诊断中的应用价值显著，准确性较高，值得推广。

Abstract：Objective To explore the application value of ultrasonic examination in the diagnosis of acute appendicitis.Methods A total of 60 patients with suspected acute appendicitis admitted to our hospital from January 2016 to march 2019 were selected as the subjects. All the patients were examined by ultrasound and computed tomography (CT), and the gold standard was surgical and pathological diagnosis. The positive predictive value, negative predictive value, accuracy, sensitivity and specificity of the two methods were compared. Results The diagnosis of acute appendicitis was positive in 55 cases and negative in 5 cases. The accuracy of ultrasonography was 85.00% (51/60), higher than that of CT (70.00% [42/60]), and the difference was statistically significant (P＜0.05). The sensitivity (85.45% [47/55]), specificity (80.00% [4/5]), positive predictive value (97.92% [47/48]) and negative predictive value (33.33% [4/12]) of ultrasonography were not statistically significant compared with those of CT (72.73% [40/55], 40.00% [2/5], 93.02% [40/43]and 11.76% [2/17]) (P＞0.05). Conclusion The application value of ultrasonography in the diagnosis of acute appendicitis is significant, its accuracy is high, and it is worth popularizing.
