高频超声在小儿肠系膜淋巴结炎诊断中的临床价值

摘要目的分析高频超声诊断小儿肠系膜淋巴结炎的临床价值。方法选取2016年1月～2018年6月于我院就诊的52例肠系膜淋巴结炎患儿作为研究组，选取同期于我院行健康检查的52例儿童作为对照组，以病理检查结果为金标准，评估高频超声诊断的符合率，对比两组儿童的淋巴结大小，并分析患儿的淋巴结影像学表现。结果肠系膜淋巴结炎患儿行高频超声诊断的符合率为96.15%，与病理诊断结果比较，差异无统计学意义（P＞0.05）；研究组儿童肠系膜淋巴结横径和纵径均大于对照组，差异有统计学意义（P＜0.05）；淋巴结集中于右下腹或脐周（右下腹淋巴结肿大27例、脐左侧淋巴结肿大5例、脐周淋巴结肿大20例），淋巴结呈椭圆形、形态规则、边缘清晰、包膜和淋巴门结构完整，肿大淋巴结呈散状分布且无融合；治疗后研究组患儿淋巴结横径、纵径均小于治疗前，纵横比大于治疗前，差异有统计学意义（P＜0.05）。结论高频超声诊断小儿肠系膜淋巴结炎准确率高且操作简便，医者可通过分析结节影像学特点判断疾病，进而为后续诊疗工作的开展提供参考。

Abstract：Objective To analyze the clinical value of high frequency ultrasound in diagnosis of mesenteric lymphadenitis in children.Methods Altogether 52 children with mesenteric lymphadenitis who were examined in our hospital from January 2016 to June 2018 were selected as the study group,and 52 children who underwent health examination at the same time were selected as the control group.The coincidence rate of high-frequency ultrasound diagnosis was evaluated according to the results of pathological examination.The size of lymph nodes was compared between the two groups and the imaging findings of lymph nodes was analyzed.Results The coincidence rate of high-frequency ultrasonography in diagnosis of mesenteric lymphadenitis was 96.15%,and there was no statistically significant difference in comparison with pathological diagnosis results(P＞0.05).The transverse and longitudinal diameters of mesenteric lymph nodes of the study group were larger than those in the control group,with statistically significant difference(P＜0.05).The lymph nodes were centrally distributed in the right lower abdomen or umbilical cord(there were 27 cases of right lower abdominal lymph node enlargement,5 cases of left umbilical lymph node enlargement and 20 cases of periumbilical lymph node enlargement),with ellipse shape,regular morphology,clear margin,structurally-complete capsule and lymphatic hilum as well as scattered and non-fused enlarged lymph nodes.The post-treatment transverse diameter and longitudinal diameter of lymph nodes of study group were smaller than those before treatment and the aspect ratio was greater than that before treatment,with statistically significant difference (P＜0.05).Conclusion Highfrequency ultrasound has the high accuracy and easy-to-operate features in diagnosis of mesenteric lymphadenitis in children,through which doctors can analyze the imaging characteristics of nodules to determine the disease,and then provide a reference for follow-up diagnosis and treatment.
