阿奇霉素联合甲泼尼龙琥珀酸钠治疗难治性支原体肺炎患儿的临床效果观察

摘要目的探讨阿奇霉素联合甲泼尼龙琥珀酸钠治疗难治性支原体肺炎患儿的临床效果。方法选取2015年1月～2017年5月我院儿科收治的80例难治性支原体肺炎患儿作为研究对象，依据治疗措施不同将其分为两组，每组各40例。对照组患儿采用单纯阿奇霉素治疗，观察组患儿采用阿奇霉素联合甲泼尼龙琥珀酸钠治疗。观察比较两组患儿的喘息消失时间、咳嗽消失时间、咯痰消失时间、退热时间及临床疗效情况，观察比较两组患儿治疗前后的C-反应蛋白（CRP）及乳酸脱氢酶（LDH）水平。结果观察组患儿的喘息消失时间、咳嗽消失时间、咯痰消失时间、退热时间均短于对照组，差异有统计学意义（P＜0.05）。治疗前，两组患儿的CRP、LDH比较，差异无统计学意义（P＞0.05）；治疗后，两组患儿的CRP、LDH均低于治疗前，观察组患儿的CRP、LDH均低于对照组，差异有统计学意义（P＜0.05）。观察组患儿的临床治疗总有效率高于对照组，差异有统计学意义（P＜0.05）。结论阿奇霉素联合甲泼尼龙琥珀酸钠治疗难治性支原体肺炎，患儿的临床症状改善明显，临床效果良好，值得临床推广应用。

Abstract：Objective To investigate the clinical effect of Azithromycin combined with Methylprednisolone Sodium Succinate in the treatment of children with refractory Mycoplasma pneumonia.Methods A total of 80 children with refractory Mycoplasma pneumonia treated in our hospital from January 2015 to May 2017 were selected as subjects.According to the different treatment measures,patients were divided into two groups,40 cases in each group.The patients in the control group were treated by Azithromycin alone,and patients in the observation group were treated by Azithromycin combined with Methylprednisolone Sodium Succinate were classified into observation group.The time of wheezing,cough,and sputum expectoration disappeared,the time of fever abating,clinical efficacy,and the levels of C-reactive protein(CRP)and lactate dehydrogenase(LDH)before and after therapy in the two groups were observed and compared.Results In the observation group,the time of wheezing,cough,and sputum expectoration disappeared,and the time of fever abating were all shorter compared with those in the control group with statistical significance(P＜0.05).The levels of CRP and LDH before treatment were not displayed significant differences between the two groups (P＞0.05).After therapy,the levels of CRP and LDH in the two groups were both lower compared with those before treatment,at the same time,the CRP and LDH levels after therapy in the observation group were both lower compared with those in the control group,the differences were statistically significant(P＜0.05).The total clinical effectiveness rate in the observation group was higher than that in the control group with a significant difference (P＜0.05).Conclusion Azithromycin combined with Methylprednisolone Sodium Succinate in the treatment of children with refractory Mycoplasma pneumonia can significantly improve the clinical symptoms and obtain a good clinical effect,which is worthy of clinical application and promotion.
