低分子肝素联合阿司匹林对复发性流产血栓前状态的临床价值

摘要目的探讨低分子肝素联合阿司匹林治疗复发性流产血栓前状态的临床效果。方法选取本院2013年12月～2015年12月诊治的复发性流产血栓前状态患者88例，根据治疗方案分为两组，对照组44例采用阿司匹林治疗，观察组44例采用低分子肝素联合阿司匹林治疗，比较两组相关临床指标、妊娠结局、不良反应发生率。结果治疗后，两组的D-二聚体、纤溶酶原激活物抑制物-1水平显著降低（P＜0.01），抗凝血酶Ⅲ、β-人绒毛膜促性腺激素水平显著增高，凝血酶时间显著增长（P＜0.01）。观察组治疗后的D-二聚体、纤溶酶原激活物抑制物-1水平，流产率、总不良反应发生率明显低于对照组（P＜0.01）。观察组的抗凝血酶Ⅲ、β-人绒毛膜促性腺激素水平，早产活婴率、足月产率、活产率明显高于对照组，凝血酶时间明显长于对照组（P＜0.01）。结论低分子肝素联合阿司匹林治疗复发性流产血栓前状态的效果显著，可明显改善患者的临床指征和妊娠结局，且安全性高，值得临床推广应用。

Abstract：Objective To investigate clinical effect of Low Molecular Weight Heparin combined with Aspirin in the treatment of pre-thrombotic state of recurrent abortion.Methods 88 patients with pre-thrombotic state of recurrent abortion diagnosed and treated in our hospital from December 2013 to December 2015 were selected and they were divided into two groups according to treatment plan.44 patients treated by Aspirin were selected as control group,while 44 patients treated by Low Molecular Weight Heparin combined with Aspirin were selected as observation group.Clinical related indexes,pregnancy outcome,the incidence of adverse reactions were compared between two groups.Results After treatment,D-two polymer and plasminogen activator inhibitor-1 level were decreased significantly in two groups(P＜0.01).AntithrombinⅢ,β-HCG level,thrombin time increased significantly in two groups(P＜0.01).D-two polymer,plasminogen activator inhibitor-1,abortion rate,incidence rate of adverse reactions in observation group were significantly lower than those in control group(P＜0.01).AntithrombinⅢ,β-HCG,thrombin time,preterm birth rate,term birth rate,live birth rate in observation group were significantly higher than those in control group(P＜0.01).Conclusion Low Molecular Weight Heparin combined with Aspirin have significant effect in treatment for pre-thrombotic state of recurrent abortion,which can improve the clinical indications and pregnancy outcome,which also has high safety and worthy of clinical and promotion and application.
