快速康复外科护理在腹腔镜膀胱全切术围术期中的应用

摘要目的探讨快速康复外科护理在腹腔镜膀胱全切术围术期中的临床价值。方法选取我院2013年10月～2015年10月行腹腔镜膀胱全切术患者96例，采用随机数字表法分为对照组和观察组，每组48例。对照组患者实施围术期常规护理，观察组患者实施快速康复外科护理。比较两组患者的恢复指征、术后生活质量、并发症情况。结果观察组患者术后进食时间[（7.1±1.3）h]、术后排气时间[（31.5±4.6）h]、术后排便时间[（67.9±8.1）h]、下床活动时间[（2.8±0.4）d]早于对照组[（18.3±2.0）h、（53.2±5.7）h、（82.5±7.2）h、（3.6±0.7）d]。观察组患者术后住院时间[（6.5±1.0）d]少于对照组[（9.8±1.2）d]。观察组患者术后生活质量[心理功能（85.9±7.3）分、物质生活（87.2±6.4）分、躯体功能（81.3±5.7）分、社会功能（88.0±6.1）分、总体评分（84.6±7.9）分]高于对照组[（70.2±5.8）分、（66.3±5.1）分、（69.7±9.2）分、（71.2±5.3）分、（72.4±4.6）分]。观察组患者并发症发生率（2.1%）低于对照组（14.6%），差异均有统计学意义（P<0.05）。结论快速康复外科护理在腹腔镜膀胱全切术围术期中的应用效果显著，可缩短患者的术后恢复时间，提高生活质量，且安全性高，值得临床推广使用。

Abstract：Objective To investigate clinical value of rapid rehabilitation surgery nursing during perioperative period of laparoscopic total cystectomy.Methods 96 patients with laparoscopic total cystectomy were selected in our hospital from October 2013 to October 2015,who were divided into two groups by random number table method.48 patients treated routine nursing during perioperative period as control group.48 patients treated rapid rehabilitation surgery nursing during perioperative period as observation group.Recovery indications,postoperative quality of life,complications were compared between two groups.Results Postoperative feeding time ([7.1±1.3] h),postoperative ventilation time ([31.5±4.6] h),postoperative defecation time([67.9±8.1]) h,ambulation time ([2.8±0.4] d) in observation group were earlier than control group ([18.3±2.0] h [53.2±5.7] h,[82.5±7.2] h,[3.6±0.7] d).Postoperative hospitalization time ([6.5±1.0] d) in observation group was less than control group ([9.8±1.2] d).Postoperative quality of life [psychological function:(85.9±7.3) points,material life:([87.2±6.4] points,physical function:[81.3±5.7] points,social function:[88.0±6.1] points,overall score:[84.6±7.9] points) in observation group were higher than control group ([70.2±5.8] points,[66.3±5.1] points,[69.7±9.2] points,[71.2±5.3] points,[72.4±4.6] points).Complication rate (2.1%) in observation group was lower than control group (14.6%).Differences were statistically significant (P<0.05).Conclusion Rapid rehabilitation surgery nursing have significant application effect during perioperative period of laparoscopic total cystectomy,which can shorten postoperative recovery time and improve quality of life,which also have high safety and worth of clinical use.
