腹腔镜胆道镜双镜联合治疗胆总管结石的临床效果

摘要目的探讨腹腔镜胆道镜双镜联合治疗胆总管结石的临床效果。方法选取2014年6月～2017年1月在我院接受胆总管结石治疗的156例患者，根据随机数字表法分为实验组和对照组，每组78例。实验组行腹腔镜胆道镜双镜联合术，对照组行传统开腹手术。比较两组的术后并发症发生率情况以及手术时间、出血量、住院时间和肠胃功能恢复时间等相关指标。结果两组患者均未出现结石残留。实验组的伤口感染、胆瘘、食道炎发生率分别为1.28%、0.00%和2.56%，对照组为8.97%、3.85%和11.54%，差异有统计学意义（P＜0.05）。实验组的手术时间明显长于对照组，出血量少于对照组，术后住院和肠胃功能恢复时间显著短于对照组，差异有统计学意义（P＜0.05）。结论腹腔镜胆道镜双镜联合治疗胆总管结石具有术后并发症发生率低、恢复周期短、残留结石率低等优点。

Abstract：ObjectiveTo explore the clinical effect of laparoscope combined with choledochoscope in the treatment of choledocholithiasis.MethodsA total of 156 patients who were suffered from choledocholithiasis from June 2014 to January 2017 in our hospital were selected and divided into the experimental group and the control group according to the random number table method,with 78 patients in each group.In the experimental group,combination of laparoscope and choledochoscope was used,while in the control group,traditional laparotomy was provided.The incidence rate of postoperative complications,operation time,amount of bleeding,hospital stay,and recovery time of gastrointestinal function were compared between the two groups.ResultsNo residual stone occurred in both groups.The incidence rate of wound infection,biliary fistula and esophagitis was 1.28%,0.00%and 2.56%in the experimental group,and the incidence rate was 8.97%,3.85%,and 11.54%in the control group,which was displayed a statistical difference (P＜0.05).The operation time in the experimental group was longer than that in the control group,and the other indexes of amount of bleeding in the experimental group was less than that in the control group,hospital stay after surgery and recovery time of gastrointestinal function in the experimental group was shorter than that in the control group,with statistical differences(P＜0.05).ConclusionCombination of laparoscope and choledochoscope in the treatment of choledocholithiasis has advantages of low incidence of postoperative complications,short recovery period,and low residual stone rate.
